Preparations for the treatment of trophic ulcers In recent years, on the whole planet, the increase in the incidence of diabetes mellitus has already taken on the dimensions of a large-scale epidemic of non-infectious type.
The social importance of diabetes is determined by its frequency and chronic disabling complications, including trophic ulcers, the formation of the diabetic foot syndrome.

This approximate etiological frequency of trophic ulceration is described: arterial - 14%, varicose - 52%, postthrombophleotic - 7%, mixed - 13%, "purely" diabetic - 5%, post traumatic - 6%, neutrrophic - 1%, others- 2%.But given that diabetes mellitus, especially the second type, can be diagnosed mainly at clinical stages, the frequency of trophic ulcers with diabetes mellitus is probably around 40-50%.

Methods of treatment.

The issues of improving the existing methods for treating trophic ulcers of the lower extremities are urgent, as well as finding measures that help prevent the bad consequences of this disease and the associated mortality and high disability.

A study was made of the safety and clinical efficacy of some drugs for the treatment of patients with diabetes mellitus in the form of trophic ulcers of the lower extremities of stages I-II.The safety and efficacy of the drugs, including "Fusikutan", "Delaskin", as well as "Vuluminamulin", were tested.

"Delaskin" is the only synthetic synthetic tannin known today - an astringent. The formula of the substance was developed in 1950.While "Delaskin" has all the characteristics of substances of natural origin, it exceeds them simultaneously by several parameters. First and foremost, artificial tannin is able to completely clean out impurities. Secondly, its consistency helps to ensure the most accurate dosage. Thirdly, the production of synthetic tannin in a rigid form is controlled at each stage of production. Fourth, artificial tannin has a long shelf-life and no staining parameters.

Release of "Delaskin" is made in the form of powder( 0.4%), or cream. During application to the surface of the wound, tannins bind the structural proteins and form a natural film on the upper layers with them, which helps to reduce the permeability of the skin surface. This causes inhibition of exudation of the wound, due to which a fairly rapid regeneration and drying, as well as restoration of the skin. Subsequent clinical studies, as well as test observations of patients led to the discovery of three more important useful properties of tannins. Useful properties:

  • Antipruritic effect. Most of the trophic lesions are complicated by itching, which manifests itself in the form of unpleasant sensations in the form of a burning sensation or tingling caused by irritation of nerve cells. During the testing of tannin with the use of electric pulses, a high sensitivity limit was fixed, in which patients experience unpleasant sensations, which indicates high antipruritic anesthetic characteristics of tannin.
  • Anti-inflammatory effect. Experimentally and in a number of clinical studies, it was found that tannin also has anti-inflammatory properties.
  • Antimicrobial effect. Tannin, which guarantees the interruption of exudation from the wound, is an obstacle to the reproduction of microorganisms.

Preparations for the treatment of ulcers.

Preparations for the treatment of trophic ulcers, which are based on synthetic tannins, should be included in pathogenetic complex therapy in combination with etiotropic and antibacterial agents. Delaskin helps to remove irritation, swelling and itching, helps reduce pain and suppress local inflammatory processes.

Considering preparations for the treatment of trophic ulcers, we turn to a drug such as Fusikutan. The active substance in it is fusidic acid. One gram of ointment / cream contains 20 mg of fusidic acid( in the form of fusidic acid hemihydrate).Fusidic acid is an antibiotic of Fusidium coccineum. The drug contributes to the violation of protein synthesis of the microorganism, and based on the dosage has a bacteriostatic or bactericidal effect. The drug has a significant antibacterial effect on a wide range of Gram-positive microorganisms - Streptococcus spp., Staphylococcus spp., Propionibacterium acnes and Corynebacterium minutissimum, including staphylococci, which are resistant to the effects of streptomycin, penicillin, erythromycin, levomycetin and other antibiotics. Fusicutane has the ability to work through intact skin. Fusikutan is used to treat wound infections, which are accompanied by the formation of scabs( they help to retain moisture and, thus, to soften hardened scabs of formation). The drug is applied three times a day to the affected area with a thin layer( applied in the lunch, morning and evening). The course of therapy is often about 6-8 days.

"Vuluminamulin" is an ointment for external use on the surface of wet, open or infected wet lesions. Due to the complex composition( coriander oil, water extract of wheat embryos, phenoxyethanol, lavender oil, sorbitol, polyethylene glycol 1500 and 400) has the ability to improve epithelization and wound healing. The aqueous extract of wheat germ, which is part of the vulina vulva, also has an anti-inflammatory effect. Vulginity must be applied 1-2 times on the wound itself, which is previously cleaned with a sterile liquid, the main borzo physiological solution. Apply a thin layer of ointment on the wound and on the coating around the wound. In addition, the dressing should be made from bandaging breathable materials to avoid excessive accumulation of heat and ventilation.
