Operation of root tip resection

In general, dental surgeons do not remove teeth, but carry out manipulations that allow them to be retained. Many people think that surgical dentistry usually removes the teeth, in fact, the tooth is removed only if other methods of dentistry do not help, that is, as a last resort. Most often, teeth are retained by resection of the apex of the root of the tooth. This manipulation is also called apicocectomy of the root of the tooth. This manipulation is to remove foci of infections localized at the roots of the teeth. Resection is carried out with the formation of cysts or granulomas on the tip of the tooth, as well as if there is an exacerbation of pereodontitis.

If the root canal root is blocked by a foreign object and further tooth treatment is not possible, resection of the top of the tooth root is also performed. As a rule, granulomas and cysts do not manifest themselves for a while, but this is only a temporary phenomenon, since sooner or later they will show themselves and are insidious enough. To make it easier for the doctor to get to the root canal, resection is performed on the upper jaw, more precisely on the front teeth. This allows you to seal the root of the tooth.

There are other indications in which resection is performed:

  • with a significant distortion of the channels;
  • if endodontic therapy was performed in the region of the top of the tooth root, the purpose of which was to eliminate the infection and which turned out to be unsuccessful;
  • if there is a need to treat canals in the tooth, the root of which is not available for treatment, due to the fixed non-removable prosthesis;
  • if the periodontitis has become aggravated;
  • if there is a foreign body in the root of the tooth, for example, a fragment of the endodontic instrument;
  • if perforation of the root wall is present in the apex region;
  • if the seal is sloppy and out of the top of the root;
  • if the upper third part of the tooth root is broken.

Resection is not performed:

  • with strong fracture of the crown portion of the tooth( and then, only when it is impossible to make an artificial limb on this tooth);
  • in cases where the granuloma surrounded the root of the tooth by 1/3 of its length;
  • with exacerbation of periodontitis;
  • with tooth mobility of 2 and 3 degrees;
  • for exacerbations of cardiovascular diseases and in the presence of acute infectious diseases.

The essence of the operation

The procedure described in itself is almost non-traumatic and lasts for a relatively short time. However, resection of the upper root of the tooth in surgical dentistry is a complex operation. Resection is carried out under local anesthesia, the patient is in the surgical chair, no special operating room, just like with a normal tooth extraction. Before the manipulation, the tooth is depulpated without fail, if, of course, the tooth with nerves. Depulpation is carried out even in the absence of toothache. Most often the dentist explains to the patient what will be done during resection, but what is done is this: the apex of the root of the tooth is cut off and with it the focus of inflammation is removed. During a resection near the tooth, the gum tissue is opened and all that has hit the infection has been removed. Then at the end of the canal the doctor puts a small seal and the operation ends with the application of stitches. Usually several stitches are applied, this allows the wound to heal properly. After the operation, it takes 6-8 days to remove the stitches.

Crown usually, if it is on the tooth, do not touch. In the postoperative period, the patient will feel discomfort, which will disappear as soon as everything heals. Remember, no surgical intervention is without consequences, and surgical dentistry is no exception. After resection, it is quite normal, if the patient will have a swelling, which in time will pass. The patient must follow certain rules after resection: the operated tooth at meals is not heavily loaded( the first three months);To do a repeated x-ray( after 6 months and 1 year).

The operated tooth is stored for a long time, and it is possible that there will be no problems with this tooth in the future. It is worth noting and the fact that teeth after resection can even be a support with further prosthetics.

The described method of resection of the upper root of the tooth has its drawback - because of the shortening of the root of the tooth, the functions of the tooth decrease and its anatomical integrity is destroyed.
