Rectum: diseases, symptoms, treatment, diseases of the rectum, symptoms of the anus

The rectum is the last part of the intestine, which is responsible for the excretion of feces. The process of digestion is no longer taking place here. However, there is a significant amount of nerve fibers - for the timely transmission of a signal about the overflow of the rectum.

Muscle tissue, as well as the intestinal mucosa lining this part of the intestine, experience colossal load, especially in patients with chronic diseases of the digestive system and with a tendency to constipation. In this case, the risk of rectal diseases increases significantly.

If you seek medical help in time (when the first symptoms of the disease appear), in most cases the disease can be stopped. The doctor will prescribe treatment to help the individual patient avoid recurrent episodes.

However, many are embarrassed to consult a doctor with such delicate complaints, prefer to endure or self-medicate. Alas, in these cases, the chance of recovery is rather small. And the rapid development of the disease can affect health and significantly spoil the patient's daily life. Constant pain, exhausting burning and itching, painful emptying process lead to the accumulation of nervous tension, neurosis and insomnia.

Causes of diseases of the rectum

Doctors themselves say that all the causes of rectal ailments have not been studied in detail. There is a list of situations and conditions that are likely to provoke these diseases. When several factors are present at once, the risk of developing the disease increases significantly.

Causes:

  1. Inflammation of other parts of the intestine.
  2. Frequent constipation (stool retention for more than 48 hours).
  3. A history of varicose veins.
  4. Anal fissures caused by too dense stool. This also includes other mechanical damage and ruptures of the mucous membrane.
  5. Improper nutrition (the diet is high in fat, meat, baked goods and carbohydrates against the background of a lack of vegetables and fruits).
  6. Insufficient physical activity, as a result - stagnation of blood in the small pelvis.
  7. Sexual infections.
  8. Dysbacteriosis, thrush.
  9. Ruptures during childbirth in women.
  10. Intestinal ulcer.

Important: The likelihood of developing rectal diseases increases in patients with a history of diabetes mellitus, dermatitis, allergic reactions, neurosis, painful anxiety.

Stress and anxiety lead to intestinal spasm, which in turn provokes stool retention.

Symptoms

Diagnosis of this group of diseases is complicated by the fact that at the initial stages the symptoms are practically not expressed, or they are so weakly expressed that the patient does not pay attention to them. When the signs of the disease become obvious, it turns out to be so neglected that a person needs a thorough examination and long-term treatment.

Symptoms that should alert you:

  1. Digestive upset (frequent constipation or vice versa - upset stools for no apparent reason, when diarrhea is not a sign of intestinal infection and is not accompanied by other characteristic signs).
  2. Intense burning, itching outside and inside the rectum. These sensations are especially pronounced immediately after the release of stool.
  3. Feeling not completely free from stool.
  4. Frequent and urging that does not end with defecation.
  5. Painful emptying that occurs even when stool passes freely through the rectum. The pain persists for a long time after stool.
  6. Outflow from the anus of mucous secretions mixed with blood and pus.
  7. Scarlet blood on the surface of stool or toilet paper.
  8. Sensation of a foreign body in the rectum.
  9. Uncontrolled passage of intestinal gas and feces.
  10. Pain in the lumbar region and in the abdomen below the navel. It occurs with overflow, as well as after the release of the rectum from feces.
In women, inflammation or swelling often puts pressure on the vaginal and uterine areas, resulting in pain during intercourse

Important: Even one of the symptoms listed above is enough to see a doctor. If there are several of them, a visit to the doctor should be scheduled for the near future.

Who should you contact for rectal diseases?

The examination and treatment of this group of diseases is carried out by doctor-proctologist. He prescribes diagnostic tests and procedures, interviews and examines the patient, makes a final diagnosis. Additionally, he may recommend a consultation with a surgeon or oncologist - if there are neoplasms in the rectum.

How is the appointment with the proctologist going?How to prepare for his appointment? Read more on our portal.

Survey

Mandatory for all patients with complaints of rectal discomfort are:

  1. Visual inspection and palpation.
  2. General blood analysis.
  3. General analysis of feces, as well as analysis for dysbiosis.
  4. Rectromanoscopy - examination of the rectum using an endoscopic device.

If the data from these studies are not enough to make an accurate diagnosis or they are doubtful, the doctor prescribes additional examinations: biochemical blood test, computed tomography or X-ray of the intestine, fibrocolonoscopy, examination of other parts of the digestive path.

Types of rectal diseases

All diseases of this part of the intestine can be divided into 4 main groups.

Table 1. Groups of rectal diseases

Group Diseases
Inflammatory Proctitis, paraproctitis
Mucosal damage Cracks, ulcers
Tumor Cysts, hernias, polyps, malignant formations
Vascular Haemorrhoids

Inflammation of the anus

These diseases often develop as a consequence of food poisoning, acute intestinal infection, or persistent constipation. The mucous membrane can become inflamed from overly spicy food.

Proctitis

The symptoms are as follows: painful bowel movements, a burning sensation that persists for a period from several minutes to several hours after stool. With severe inflammation, false desires appear, as well as the release of a small amount of blood, pus and mucus - immediately after stool or some time after.

Proctitis

Proctitis

Proctitis is divided into several types:

  1. Polypoid (the appearance of growths on the mucous membrane - polyps).
  2. Ulcerative leads to the appearance of erosive lesions and ulcers.
  3. Catarrhal-hemorrhagic is accompanied by multiple hemorrhages and damage to small vessels.
  4. Catarrhal-purulent form - it is characterized by purulent formations.

Paraproctitis

This is an inflammation of the edge of the anal sphincter itself and the surrounding tissues. The disease also affects the anal glands. Signs: severe pain, the formation of seals, stool disturbance towards its delay. With severe inflammation, abscesses appear, the patient has a fever.

Paraproctitis - inflammation of the rectal gland

Paraproctitis - inflammation of the rectal gland

Paraproctitis is more susceptible to men who lead a sedentary lifestyle against the background of the abuse of carbohydrate foods and fast food, or patients with diseases of the genital organs, especially prostatitis.

Mucosal damage

A rupture of the mucous membrane can provoke too dense feces, inaccurate setting of an enema, the use of foreign objects during sexual intercourse.

In women, tears happen during childbirth.

In women, tears happen during childbirth.

A shallow crack gives the patient significant discomfort. This is one of the most common forms of anal diseases. Under favorable circumstances, a crack a few millimeters in size will heal itself. If the crack is more than a centimeter long, the patient needs medication.

Deep cracks are treated with surgery

Deep cracks are treated with surgery

Deep injuries and ruptures are more dangerous: they often lead to infection in the peritoneum and inflammation of internal organs.

Signs:

  1. Pain directly in the anus, as well as in the abdomen.
  2. Flatulence, hard stomach.
  3. Temperature increase.
  4. Difficulty breathing.

In severe cases, internal bleeding and peritonitis (inflammation of the peritoneum) develop. These patients need urgent surgical care. If it is not provided, the patient will face a painful death in the period from several hours to several days.

Cysts

These are benign formations that manifest themselves as a result of impaired intestinal development even during intrauterine development. Also, a cyst is a complication of a prolonged inflammatory process in the rectum. The disease does not manifest itself for a long time, especially if the size of the cyst is small. When it grows, the patient begins to worry about the symptoms:

  1. Heaviness and pressure in the anus, aching pain.
  2. Stool disorder (alternating diarrhea and constipation).
  3. Gas incontinence. If the cyst is very large, fecal incontinence occurs.

Important: If the cyst is damaged and infected, suppuration may develop, which is accompanied by acute throbbing pain and discharge of pus. The body temperature rises, signs of intoxication and inflammation appear. In this case, you need to see a doctor as soon as possible.

Even in the absence of inflammation and suppuration, an untreated cyst can lead to fistula formation.

Symptoms of rectal fistulas

Symptoms of rectal fistulas

Hernia

The second name of this disease is rectocele. It occurs due to weakness or rupture of the pelvic floor muscles (during childbirth in women, as a result of surgical operations on the abdominal organs). A hernia can be a complication of chronic constipation, hemorrhoids, excessive physical exertion. The main symptoms are: aching pain, heaviness in the anus, prolonged stool retention and painful bowel movements, secretion of blood and mucus. In women, the protrusion of the hernia towards the vagina causes pain during intercourse and urination, pain in the lower abdomen and lower back, which are often mistaken for signs of gynecological diseases.

Polyps

Polyps are benign growths that often do not cause any discomfort. The patient feels pain and difficulty in defecation if the polyp is very large or there are several formations. In this case, an operation is recommended. removal of education.

Rectal polyp

Rectal polyp

Symptoms of the presence of polyposis:

  1. Aching pain in the lower back and lower abdomen.
  2. Unsuccessful urge to empty.
  3. Stool retention followed by diarrhea.
  4. Leakage of blood and mucous contents.

Cancer

Cancers often develop against the background of pre-existing bowel cancer or as a complication of untreated diseases of the rectum, especially inflammation and polyps. The main difficulty lies in the fact that the disease does not manifest itself for a long time. The patient is disturbed by the maximum sensation of a foreign body and a small amount of blood during bowel movements - the same signs that indicate a number of other diseases and are ignored by patients to the last.

Rectal cancer

Rectal cancer

At later stages, there are:

  • frequent stool disorders - prolonged constipation alternates with diarrhea;
  • fever, temperature rises;
  • rapid weight loss;
  • severe pain that occurs between bowel movements;
  • the release of large amounts of blood and mucus. The volume of daily blood loss can reach 200 ml, against the background of which anemia and exhaustion develop.

Important: Metabolic disorders, weakness, dysfunction of the genitals are frequent complications of advanced stages of rectal cancer.

Diseases associated with vascular lesions

Varicose veins in the lower intestine lead to hemorrhoids. The disease has three main causes: heredity, lack of activity, and malnutrition. The main signs of hemorrhoids:

  1. Redness and swelling of the anus.
  2. Unpleasant painful sensations during bowel movements.
  3. Feeling of incomplete cleansing of the rectum, the presence of a foreign object in it.
  4. Violent piercing pain after stool.
  5. Allocation of a small amount of mucous membranes and blood impurities.
  6. If the disease is not treated, the nodes fall out, causing severe pain and discomfort.
Haemorrhoids

Haemorrhoids

Hemorrhoids are external and internal. The external one is easy to detect upon visual examination of the patient. Internal diagnosed using endoscopic methods or rectal examination. The earlier a person comes to see a proctologist, the easier and faster the treatment process will be. The early stages of hemorrhoids are well treated using conservative methods, and if further recommendations are followed, relapses can be completely excluded. In the later, surgical intervention is indispensable. The appearance of hemorrhoids indicates that the disease has already started.

Which doctor should I go to for hemorrhoids? What are the most effective drugs in the fight against this ailment? Are there any nutritional advice for hemorrhoids. You will find answers to these questions on our portal.

Rectal prolapse

Often this condition is a consequence of hemorrhoids, but it can also occur on its own. Part of the mucous membrane falls out. This happens during a bowel movement or with the slightest effort and physical exertion.

Rectal prolapse

Rectal prolapse

There are three stages of loss:

  1. The first is characterized by episodic prolapse during bowel movement, especially after stool retention. After a while, the dropped out part returns to its original position without additional help.
  2. In the second stage, the loss can provoke physical effort - for example, lifting weights, exercise. The prolapse is not eliminated on its own, it must be corrected. After that, the repeated loss does not occur.
  3. In the third section of the rectum falls out even without physical activity, just in a standing position. After reduction, immediately or within a few minutes, the rectum falls out again. This condition is usually accompanied by fecal and gas incontinence, bleeding.

Important: The more often loss occurs, the greater the likelihood of ulcers and bleeding foci on the mucous membrane.

The help of a proctologist with rectal prolapse is necessary as soon as possible.

Treatment

For diseases of the rectum, conservative and surgical methods are used. Folk remedies are used as an adjunct to the main treatment. Also, the dietary nutrition and lifestyle of the patient play an important role.

Inflammatory diseases and shallow cracks in the mucous membrane are treated with medication. For this, local remedies are used - candles, ointments, gels, microclysters. With severe inflammation, antibiotics and pain relievers are required.

Neoplasms, especially malignant ones, are treated by surgical removal. This process is accompanied by drug treatment.

For acute pain and difficulty in emptying the bowels, laxatives (local or general), as well as pain relievers, are used. Anesthetic suppositories, which are inserted into the rectum after a bowel movement, help to quickly relieve pain.

Drug treatment of rectal diseases

For conservative treatment, medications are used:

  1. General action - tablets, less often - injections. In this form, antibiotics, venotonic and immunostimulating drugs are prescribed. Taking pills helps to significantly enhance the effect of local treatment and accelerate recovery.
  2. Local remedies - candles, ointments, microclysters. These can be anti-inflammatory, antibacterial, healing, pain relievers, and laxatives.

Table 2. Drug treatment of rectal diseases

Diseases Treatment
Proctitis, paraproctitis
  • antibiotics (Clarithromycin, Ciprofloxacin);
  • local antibacterial drugs (microclysters with a solution of furacillin, rivanol);
  • light laxatives (Duphalac, Lactulose);
  • anesthetics (Betiol, Anusol);
  • drugs that accelerate the regeneration of the mucous membrane (Relief);
  • drugs based on hydrocortisone and prednisolone - to reduce severe inflammation;
  • antiallergic - to eliminate puffiness (Tsetrin, Fenistil, Claritin);
  • probitic drugs to improve intestinal microflora (Bifiform, Linex).
  • Cracked anus For the treatment of anal fissures, the same drugs are used as in the inflammatory process:
  • light laxatives - for oral administration and local in the form of suppositories and microclysters;
  • antibacterial drugs - for the prevention or treatment of an associated infection;
  • wound healing agents (Levomekol, Relief, nitroglycerin ointment, Methyluracil suppositories);
  • pain relievers - Anestezol, Proctoglivenol;
  • microclysters with sea buckthorn oil or sea buckthorn candles simultaneously relieve inflammation, promote healing of the mucous membrane.
  • Haemorrhoids Local remedies are used to relieve pain and speed up the resorption of hemorrhoids. Tablets are prescribed to improve vascular tone.
    The main medicines used:
  • anticoagulants - drugs based on heparin;
  • hemostatics - are needed to reduce and stop bleeding. In this regard, Natalsid rectal suppositories have proven themselves well;
  • venotonics, which strengthen the walls of blood vessels and reduce their permeability (Proctoglyvenol, Troxevasin, Detralex, Phlebodia, etc.). These drugs are given in pill and topical form;
  • anesthetics (lidocaine in the form of a spray);
  • anti-inflammatory drugs.
  • Cyst Medicines are used only to reduce inflammation and pain during suppuration - these are antibiotics, pain relievers, and anti-inflammation drugs. This treatment is temporary and is used to prepare the patient for surgery. The main treatment for cysts is surgical removal only.
    Hernia Conservative treatment is effective only in the first stage. General and local antibacterial, analgesic, antispasmodic drugs are used, as well as gymnastics to strengthen the muscles of the pelvic floor.
    The second and third stages do not lend themselves to conservative treatment; in this case, only the operation will help the patient.
    Sometimes patients with proctitis (if it is due to chronic stress) are prescribed sedatives and psychotropic medications

    Sometimes patients with proctitis (if it is due to chronic stress) are prescribed sedatives and psychotropic medications

    Important: Laxatives should not be used in patients with irritable bowel syndrome..

    What does traditional medicine advise?

    There are many alternative methods for getting rid of rectal diseases. It is advisable to use them as an addition to the main therapy.

    For the treatment of proctitis and paraproctitis:

    1. Anti-inflammatory ointment is prepared on the basis of natural ingredients: water pepper, toadflax, oak bark, pork fat. The mixture of herbs must be chopped (passed through a meat grinder or blender), filled with melted fat. Let it brew for 12 hours. Then dip a clean gauze swab in the ointment, insert into the rectum and leave for at least 5 hours. The duration of the procedures is 21 days.
    2. Microclysters based on herbal decoctions will help to quickly relieve inflammation. They need to be done after bowel movement. The infusion should be kept in the intestines for several hours, and preferably at night. For microclysters, decoctions of chamomile, calendula (separately or a mixture), oregano and lemon balm are used. The same decoctions can be used for sitz baths. Depending on the intensity of the inflammation, the treatment will take 2-3 weeks.
    3. An aqueous infusion of calendula for oral administration is prepared at the rate of a tablespoon of dry herb in a glass of boiling water. Insist for several hours, then drink 2 tablespoons 2-3 times a day for a month.
      Calendula decoction

      Infusion of calendula

    4. Sitting baths with horsetail are taken for 2-3 weeks. To prepare a decoction, half a glass of dry raw materials need to be poured with a liter of boiling water, insist for 10-15 minutes. The duration of the procedure itself is 20 minutes. It is important not to allow the bath to cool down too much by adding hot broth or water.

    Anal fissure healing:

    1. Candles based on beeswax and crushed herbs of chamomile, plantain and motherwort will help to quickly remove inflammation and accelerate the regeneration of the mucous membrane. Pour the herbal mixture with melted wax. While it is still warm, mold a small candle. You can store them in the refrigerator. After emptying the intestines, inject 1 pc. into the anus (no more than 3 times a day) for 2 weeks.
    2. You can also do sitz baths for the anal area. For the baths, you can use a decoction of onion peels, a solution of potassium permanganate, decoctions of herbs - chamomile, eucalyptus, celandine. The course is 15-20 days.
    3. Micro enema with warm (heated to 37 degrees) olive or sea buckthorn oil. The volume of the enema is 30-50 ml. Oil should be introduced after bowel movement and delayed for several hours. If, after microclysters, emptying of the rectum occurs, repeat the introduction of oil. The duration of treatment is 10-14 days.
      Sea buckthorn oil has healing properties and is excellent in the treatment of gastrointestinal erosions.

      Sea buckthorn oil has healing properties

    4. It is useful to take a decoction of anti-inflammatory herbs for two weeks. The collection based on burdock root, chamomile and walnut leaves will help heal the anal fissure, at the same time improve the functioning of the entire intestine. Take 3-4 tablespoons of the mixture per liter of boiling water. You need to drink once a day in the amount of half a glass, preferably on an empty stomach.

    How to get rid of hemorrhoids using traditional medicine:

    1. A decoction of nettle will help reduce bleeding with hemorrhoids. 2-3 tablespoons of dry leaves need to be poured into glasses of boiling water and insisted for several hours, stirring from time to time. Drink 50-70 ml three times a day on an empty stomach.

      Nettle decoction

    2. A gauze swab soaked in sea buckthorn oil is inserted into the rectum at night. It helps to relieve inflammation, tone blood vessels, remove swelling and bleeding. You can also mix oil with beeswax and make a kind of rectal suppository - but only if you are not allergic to bee products.
    3. A decoction of mullein herb will help with hemorrhoids. It can be brewed with boiling water and drunk like regular tea, but no more than 3 times a day for a month.
    4. Dogwood berries should be eaten in the morning on an empty stomach, a tablespoon at a time, chewing thoroughly. You can take food after half an hour. The same amount is taken before dinner. At night, the dosage is reduced to 1 teaspoon. You need to be treated in this way for two weeks. To enhance the effect, exclude meat food, spices, completely abandon bad habits.

    For polyps in the rectum:

    1. To eliminate polyps in the straight and other parts of the intestine, a decoction of viburnum is used. Pour 200 g of chopped berries with a liter of water, bring to a boil, then drink a glass in the morning on an empty stomach. The finished broth can be stored in the refrigerator for several days. But it is better to cook fresh every 2-3 days. The course of treatment lasts a month.
      Viburnum decoction

      Viburnum decoction

    2. A decoction of celandine (a tablespoon per glass) is used for microclysters (50-70 ml is injected into the intestines 2 times a week). To enhance the effect, the same solution (diluted with water 2-3 times) can be drunk twice a day before meals for 10 days.
    3. For 2 weeks, every evening, inject a tampon moistened with a mixture of camphor oil (a tablespoon), honey (a teaspoon) and iodine (6 drops) into the rectum. After a two-week break, the course can be repeated.
    4. With a strong polyposis, microclysters based on herbal decoction will help. You will need a mixture of 3 parts meadowsweet and celandine and 2 parts of calendula, St. John's wort and agrimony. Pour a tablespoon of the mixture with 300 ml of boiling water, leave for several hours. Inject 50 ml at night for a month. If necessary, repeat the course after 2 weeks.

    Important: In the presence of oncological pathology, it is not recommended to be treated with alternative methods. In this case, it is necessary to undergo chemotherapy and surgical removal of the tumor. Relying on popular advice, the patient risks losing valuable time and seriously starting the disease.

    A cyst and a hernia of the rectum also cannot be completely cured with folk remedies. Anti-inflammatory methods - the same as for treating proctitis - can be used to relieve unpleasant symptoms. The main treatment is performed surgically.

    Prevention

    It is much easier to prevent the development of the disease than to get rid of it. But preventive measures require the patient to carefully review their lifestyle and habits. First of all, you need:

    1. Review your diet. It is necessary to include in it more food containing fiber - it stimulates peristalsis and prevents the development of constipation.
      A large amount of fiber is another reason for the active work of intestinal motility.

      A large amount of fiber stimulates intestinal motility

    2. Increase the amount of fluid consumed, if there are no strict contraindications. Water is absorbed in the intestines. If the body lacks fluid, it draws it out of the stool. As a result, the latter become very dense, and when moving along the lower parts of the intestine, they injure the mucous membrane.
      It is necessary to drink a sufficient amount of water per day.

      It is necessary to drink a sufficient amount of water per day.

    3. Do not deliberately delay the bowel movement - for example, when you are not at home, or too busy with work. Postponing this process for later leads to the accumulation of feces in the rectum, its stretching and rupture of the mucous membrane.
    4. Eliminate excessive effort when emptying the intestines. If this process is accompanied by pain, requires a lot of effort - you need to adjust the diet, as well as use special means to facilitate bowel movements. Oil micro enema as a one-time procedure will not harm and will not cause addiction, but will help prevent the appearance of an anal fissure or rupture of blood vessels.
    5. Increase physical activity, especially if you spend most of the day sitting.
    6. Carry out hygiene procedures correctly. If you already have problems or you have already been treated for rectal diseases after having a bowel movement you need to wash the anal area with cool water and soap, and not be limited to using a toilet paper. It is also worth switching to soft paper or wet hypoallergenic wipes (for example, baby wipes).
    7. Any intestinal upset or food poisoning should be treated until symptoms disappear completely.
    8. When the first signs of rectal disease appear, you should immediately consult a doctor, do not start the disease.
    Compliance with these rules will help preserve the quality of life, save you from serious health problems.

    Compliance with these rules will help preserve the quality of life, save you from serious health problems.

    It should be remembered that the appearance of diseases of the rectum largely depends on an improper lifestyle: abuse of unhealthy and spicy food, alcohol consumption, lack of mobility. If you instill healthy habits, these unpleasant diseases can be avoided.
