What is gallbladder cancer

Gallbladder cancer is a malignant tumor that develops from the tissues of the gallbladder. Among tumor neoplasms of the digestive system, it ranks fifth, according to some sources - 6th place.

The risk group includes patients with gallstone disease, polyps, gallbladder cysts, chronic infection (giardiasis, echinococcosis, H. pylori lesion), ulcerative colitis.

The risk increases with burdened heredity and prolonged contact with unfavorable factors. These include occupational hazards, contact with chemical carcinogens. The list continues with bad habits (alcohol abuse, smoking), eating disorders, eating food containing carcinogens.

Ultrasound

The essence of the study is to create an image of an organ by scanning it with ultrasonic waves. Determination of the size, wall thickness, contents and shape of the gallbladder is carried out. These studies are compared with the indicators accepted as the norm. As a rule, not only the bladder and bile ducts are scanned, but also the liver.

When examining the gallbladder, the pathological process may indicate:

  • thickening of the wall;
  • inclusions visualized in the bladder cavity (calculi, pus, bile, more dense formations);
  • tumor mass is determined at the site of the gallbladder

Ultrasound of the gallbladder

The detection of a tumor by classical methods of ultrasound diagnostics is accompanied by difficulties. Therefore, more sophisticated technologies are used in the research process. Three-dimensional ultrasound imaging in angiography mode has several advantages:

  • allows you to refuse intravenous administration of a contrast agent and avoid possible complications (anaphylactic shock);
  • gives a more accurate result;
  • reveals the vascular pattern of tumor formation.

The results obtained are not a basis for a diagnosis. Interpretation of data is considered in conjunction with other studies.

Ultrasound does not always allow you to accurately make such a diagnosis.

Tomography

Computed tomography is a method of layer-by-layer examination of internal organs using X-rays.

The computer program sets the direction of the beam, covering various sections, determines the "step" of the study (from mm to cm). The resulting information is printed as images. When diagnosing a malignant lesion of the gallbladder, CT is used in the initial examination, as well as when specifying the stage of the process.

CT scan reveals an increase in lymph nodes, the spread of the process to the liver, metastasis to other organs. More accurate information is provided by a CT scan with the introduction of a contrast agent. The technique is prescribed separately, and more often in conjunction with other studies.

MRI is a method of creating detailed images of an organ using radio frequency electromagnetic fields. The value of the study consists in obtaining images with high contrast between different soft tissues, the ability to examine different sections. A positive difference from the previous method is the absence of ionizing radiation and associated risks and restrictions.

Magnetic resonance cholangiopancreatography (MRCP) is a specialized method for studying the pathology of the gallbladder and biliary tract. It is the most accurate screening tool. The non-invasiveness of the method is an added advantage.

Cholangiography

Cholangiography is a method of examining the bile ducts by X-ray radiation after the administration of a contrast agent. Imaging can help clarify the cause of the bile duct blockage. The contrast is injected directly into the study area. This technique is carried out during the operation.

Endoscopic retrograde cholangiopancreatography is performed using a combination of endoscopic and radiographic techniques. In the descending part of the duodenum is the Vater nipple. Through it, bile and pancreatic secretions enter the small intestine. With the help of an endoscope, the nipple of Vater is cannulated and a contrast agent is injected into it. The result is a series of images. The method is invasive, and is an option for clarifying the diagnosis and carrying out medical procedures.

Cholangiography

Percutaneous transhepatic cholangiography is performed under ultrasound guidance under aseptic conditions. A puncture is achieved with a special needle and a cannula is inserted into the bile ducts. The introduction of contrast allows you to visualize the area of ​​interest. The method is often a step in the operation. Potential complications: bleeding, infection.

Laparoscopy

Laparoscopy refers to minimally invasive surgery. The procedure is performed using an endoscopic technique, which is inserted through small incisions into the abdominal cavity. Previously, carbon dioxide is injected into the abdominal cavity. The laparoscope is equipped with a light source, video camera and fiber optics. The camcorder enlarges the image and transmits it to the monitor screen. Manipulators inserted through separate incisions allow the surgeon to view organs and perform surgical manipulations.

Determination of the stage of development

Treatment is prescribed after examination and staging of the process according to the TNM classification.

The system is based on alphanumeric encryption of the dynamics of tumor development:

  • T is the state of the primary tumor;
  • N - changes in regional lymph nodes;
  • M - presence of distant metastases.

Treatment is prescribed after a complete examination, an accurate diagnosis, and determination of the stage of the cancer.

The set of signs allows you to determine the stage of the malignant process.

Stage 1 characterized as the onset of the disease. The primary tumor does not extend beyond the inner layer of the gallbladder wall. Regional lymph nodes are not involved, there are no metastases to other organs.

Stage 2 indicates that the primary tumor has spread to the muscle layer and connective tissue. No invasion outside the organ was detected.

Stage 3 - the spread of the primary tumor outside the bladder. In the case of "A" the nearby lymph nodes are intact, in the case of "B" they are involved in the process.

Stage 4 characterized by invasion of the liver or other organs.

Stage "A" - involvement of the vascular bundle of the liver (portal vein and hepatic artery) or two or more organs. Stage B is defined by the presence or absence of metastases and regional lymph node involvement.

Treatment

The treatment tactics are determined by the stage of the process, the initial state of the patient, and the structure of the tumor. It can be invasive (surgical) or conservative.

Surgical interventions, in turn, are divided into radical and palliative.

Radical surgery - extended cholecystectomy. In the postoperative period, chemotherapy and / or radiation therapy is performed.

Palliative (symptomatic) surgery is designed to improve the patient's quality of life, but cannot stop the disease.

Chemotherapy is a treatment that uses drugs that stop tumor growth or damage cells. The main goals of chemotherapy are:

  • preparatory period before surgery;
  • prevention of metastasis and relapses after surgical treatment;
  • combined use with other types of therapy;
  • method of treatment for inoperable cases.

Chemotherapy can be given before surgery to stop the tumor from developing and literally kill it.

The method has side reactions: nausea, vomiting, weakness, appetite and sleep disorders.

Radiation therapy uses ionizing radiation to damage tumor cells. Modern devices are equipped in such a way that the shape of the irradiated volume is as close as possible to the shape of the tumor. This protects healthy tissue from ionizing radiation.

All methods can be combined or applied sequentially.

Radiation therapy may also be given before surgery to stop the spread of the tumor.

The success of any treatment tactics depends on the psychological state of the patient, his desire to cooperate with medical specialists. Any disease, especially oncological, makes a person feel shock, causes fear of death, uncertainty, helplessness, dependence. In the course of treatment, even the most persistent and strong-willed people may doubt themselves, their strengths. Without support from the outside, such a burden is very difficult to endure.
