Description and stages of thrombosis

This can lead to hemorrhoidal thrombosis and inflammation. These processes refer to the development of acute hemorrhoids, affecting internal and external hemorrhoidal cones.
The pathological process proceeds through 3 stages of development.
Stage 1
The manifestation of the acute phase of hemorrhoids proceeds without an inflammatory process. The patient feels a burning sensation, itching, pain during bowel cleansing.
Irritation in the anal area increases with the use of unhealthy foods: salty, spicy, fried, alcohol.
Physical activity plays an important role, after which the patient's condition worsens, the symptoms of the disease intensify.
The vessels in the rectum overflow with blood, hemorrhoidal cones are felt. Lack of proper treatment leads to inflammation and thrombophlebitis.

Stage 2

Stage 3

Causes

A pathological process develops, spreading to the anal area, subcutaneous fat.
The thrombosed hemorrhoid is inflamed. What affects the formation of blood clots:
- improper diet;
- insufficient fluid intake, dehydration;
- an increased level of pressure in the peritoneum (observed during pregnancy);
- colds, hypothermia;
- with abdominal surgery;
- bad habits.
Blood clots form as a result of damage to the walls of blood vessels. This entails the occurrence of thrombosis of the hemorrhoid.
With hemorrhoids, due to frequent high pressure in the veins of the rectum, changes are observed - they stretch, the walls become thinner. Blood circulation is impaired, leading to complications.
Diagnostics
External thrombosis is diagnosed by visual examination at a proctologist's appointment.
Examination of the anus reveals edema masking thrombosis. It does not look like dropped bumps.
With postpartum thrombosis, extensive swelling around the anus is noted. Due to the severe pain syndrome, palpation and proctoscopy are not performed.
This pathology in most cases ends favorably. The tissues of the node die off or a thrombus spontaneously breaks out.
Bleeding opens, an improvement in the patient's well-being is noted. The swelling subsides, the pain goes away.
With prolonged external thrombosis, hemorrhoidal fringes may remain - characteristic traces.
They are formed by stretching the epithelial tissue around the anus. They can bother you with manifestations of itching, interfere with hygiene procedures. Removal under local anesthesia is recommended.
Treatment
With this pathology, immediate treatment is required. There are two types of therapy:
- conservative;
- surgical intervention.
Acute thrombosis requires urgent hospitalization. It is forbidden to independently adjust the thrombosed node.
These manipulations lead to damage to the mucous membrane, the opening of bleeding, the development of dangerous complications.
The patient is prescribed bed rest, warm baths with potassium permanganate to relieve inflammation and pain.
Cool lead acetate lotions are made every 30 minutes. Anticoagulants, dietary food based on dairy and plant products are prescribed. Take vaseline oil inside 1 tbsp. l. daily.
Conservative treatment
Drug therapy is prescribed. It includes safe and effective drugs. These include Detralex.
It has an anti-inflammatory effect, strengthening blood vessels. Additionally, multicomponent ointments are used - Heparin, Hepatrombin G, Troxevasin.
Rectal suppositories do not have the desired effect in this case.
This type of therapy is used for not too extensive and painful thrombosis. With a decrease in the general symptoms of an acute form of hemorrhoids and inflammation, decisive action can be taken with the help of surgeons.
The most effective and fastest way of surgery is thrombectomy.
Surgical intervention
The thrombectomy procedure is performed under local anesthesia. An opening of the hemorrhoid is performed, a thrombus is removed.
An incision is made, blood clots are removed. The bumps decrease in size, the pain goes away. The operation is carried out quickly.
After the operation, the patient is allowed to go home after consulting and giving precise recommendations.
The resulting wound disappears in a matter of days. Sometimes the entire node is removed. This will prevent further manifestations of relapses.
With advanced hemorrhoids during an exacerbation, a radical surgical intervention is performed in order to prevent the recurrence of complications.
There are modern and invasive methods of thrombosis therapy:
- Laser therapy. Effectively removes large external knots. The procedure is performed under local anesthesia and takes about 1 hour. The advantage of the operation is bloodlessness and high efficiency. Painful sensations pass after the operation, there are no deformations in the anus. After the procedure, the patient can lead a normal life.
- Ligation of knots. Using special latex rings, hemorrhoidal nodules are squeezed. The procedure is applicable at stages 2-3 of the pathology. The surgeon works with a special device - an anoscope. With its help, access to the cones is discovered. With the help of a ligator, latex rings are applied, pinching the knot, blocking access to food. Necrosis gradually develops, the nodule disappears on its own.
- Infrared coagulation. With the help of a coagulator, powerful infrared rays are applied to the affected area. The mucous membrane near the nodule is burned out, a small scar is formed. It blocks the blood supply to the nodule, it is exhausted, it dies.
