What is pancreatitis
By pancreatitis, it is customary to mean a whole complex of inflammatory processes that develops in the pancreas of a particular patient.
Inflammation can be both acute and chronic, and causes significant harm to the specified organ.
Pathology leads to the fact that the enzymes that the gland produces begin to work not in the right place, but in the gland itself, provoking its self-digestion.
After that, the enzymes, along with the blood, disperse throughout the patient's body and cause a strong and dangerous intoxication of the body.
Most often, pancreatitis is formed due to the presence of stones in the gallbladder and excessive drinking.
In addition, overeating, colds, herpes, appendicitis, or food allergies can be a provoking factor.
The disease manifests itself with such symptoms: pain in the abdomen, stools of a mushy consistency, nausea with vomiting, increased sweating, tachycardia, hypotension, collapse and shock.
Pancreatitis is treated with drug therapy, folk remedies and adherence to a drug diet.
The most important thing is to remember that the chronic form of the disease in question is quite dangerous for the body, because it can cause an oncological process in the pancreas.
Pharmacological features of the drug
Contrikal belongs to a group of drugs that affects blood formation and the work of pancreatic enzymes. It is a hemostatic and inhibitor of proteinolysis.
The drug can remove bleeding caused by an overabundance of medications and agents that fight venous thrombosis. In addition, it eliminates the processes that damage the gland during pancreatitis.
Contrikal in pancreatitis controls the enzymes secreted by the gland and required for normal digestion.
During the course of pancreatitis, the body produces excess enzymes, so they digest the gland itself.
Such phenomena require urgent treatment with proteinase inhibitors, which is the agent in question.
The action of the drug, as a hemostatic, is to break down blood clots. This process is called fibrinolysis.
If it increases, then there is poor blood clotting and various kinds of bleeding.
This can be caused by pathological conditions or taking specific medications that thin the blood. It is with these types of bleeding that Contrikal fights.
What does the drug consist of and how is it produced
Currently, Contrikal has one form of release - a substance used in the preparation of a solution for intravenous administration. That is, in ampoules. The drug is not available in the form of tablets.
Aprotinin is considered the active substance, and mannitol comes to its aid.
One ampoule of the drug in question contains so much aprotinin that its activity is equal to 10,000 units of the antitrypsin type.
In pharmacies, Kontrikal is dispensed with a saline solution placed in a sterile container.
The ampoule contains 2 milliliters of such a solution - that is how much is required to prepare a bottle of Contrikal.
One package contains 10 vials of active ingredient and saline solution.
It is recommended to store Contrikal in a ventilated place, hidden from direct sunlight. The storage temperature should not exceed 26 degrees Celsius.
How does Contrikal work in pancreatitis?
The drug in question has a large number of advantages. They are as follows:
- Prevents proteins from being degraded.
- It has a positive effect on fibrin fibers in tissues.
- Stabilizes the metabolism in the pancreas.
- Normalizes hemostasis.
- Reduces the activity of enzymes of the organ in question, which can provoke the development of pancreatic necrosis.
With an exacerbation of pancreatitis, trypsin, chymotrypsin and kallikrein are intensively produced in the body.
The drug in question is able to inhibit them, having antienzyme effects, and prevent the self-digestion of organ tissues.
When is Contrikal prescribed
Contrikal is considered a universal remedy, which is usually prescribed as a drug that treats many pathologies.
You should consider them in more detail:
- Surgery on the prostate or lung.
- Amniotic fluid embolism.
- Injuries.
- Bleeding before, during and after labor.
- Conditions resulting from extracorporeal circulation.
In these cases, Contrikal is used to stop bleeding. In addition, the drug is usually used in the treatment of a number of pathologies:
- Necrosis of the gland.
- Acute pancreatitis.
- A chronic form of pancreatitis, which periodically worsens.
- Increased fibrinolysis, provoking severe bleeding.
- Acute mumps after surgery.
- Quincke edema.
- Severe tissue damage.
- Shock states of a different nature.
- With a large area of tissue damage.
As a preventive measure, Kontrikal is used in such situations:
- Self-digestion of the gland caused by surgery (postoperative pancreatitis).
- Embolism after surgery.
- Various kinds of bleeding.
- Fat embolism when the body or skull is injured.
In addition, Contrikal is prescribed in order to prevent blood loss and reduce the volume of blood for transfusion during coronary artery bypass grafting.
Side effects of Contrikal
Treatment with the drug in question can provoke some unwanted body reactions.
Most often we are talking about an allergic process and a local reaction. If the agent is re-injected, then side effects occur more often by 5%.
The most common negative consequences of taking the drug include the following phenomena:
- Lowering blood pressure.
- Increased heart rate.
- Frequent nausea with vomiting.
- Consciousness problems.
- Psychoses.
- The onset of hallucinations.
- Increased blood creatinine levels.
- Allergy.
- Local thrombophlebitis in the place where Contrikal was injected: edema, redness of the skin and pain in the area of the skin puncture.
The normalization of creatinine levels occurs on its own after discontinuation of the medication and does not need to be adjusted.
Nausea and vomiting are more likely to be triggered by injecting the drug too quickly, so medical personnel should do this more slowly.
An allergic reaction can be mild or significant. Its development is indicated by the following symptoms:
- Redness of the skin.
- Itching.
- Rash.
- Spasm of the bronchi.
- Swelling of the pharynx.
- The acceleration or stop of the heartbeat.
- Hypotension.
- Quincke's edema.
- Anaphylactic shock.
If an allergy occurs, taking the drug should be immediately canceled and replaced with analogues.
Contraindications to the drug
The agent in question has absolute and relative contraindications, which should be taken into account when prescribing Contrikal to patients.
The absolute contraindications are as follows:
- The period of bearing a child.
- Lactation.
- Hypersensitivity to animal proteins and other components of Contrikal.
- DIC - syndrome.
The relative contraindications are as follows:
- Possible hypersensitivity.
- History of allergic diathesis.
The first group of contraindications means the complete exclusion of the use of the agent in question.
And in the presence of relative contraindications, the patient should be constantly under the supervision of the attending physician.
How to take Contrikal correctly
With the initial acute pancreatitis, the drug should be taken in the following dosage: initially, the patient is injected with 300 units of the drug. The drug should be administered by drip or jet.
Maintenance dosage - 20-30 thousand units hourly, but without exceeding the above norm.
When a person's condition is normalized, the dosage is reduced to 200 thousand units and administered until the patient recovers completely.
During an exacerbation of the chronic form of the disease, the dosage changes: the initial amount of the injected drug does not exceed 50 thousand units. The same dosage is dripped for about 6 days.
In order to prevent the traumatic type of pancreatitis, Contrikal is administered as follows:
- Before surgery - 200 thousand units.
- After its completion, the drug is administered at 100 thousand every 6 hours for two days.
To mix the correct solution for the dropper, you need to add the saline solution that is sold with it to the container with the drug.
Initially, Contrikal is mixed with 2 milliliters of saline, then with some type of infusion solution. Dosages in this case should be prescribed only by a qualified specialist.
It should be remembered that immediately before using Contrikal, physicians must make a sample to the patient to determine the level of sensitivity.
If the reaction has not manifested itself, then the agent can be administered by giving the person any antihistamine for reinsurance.
Physicians, on the other hand, must follow the instructions regarding the introduction of Contrikal: no faster than 5 milliliters per minute.
No cases of overdose with Kontrikal were recorded. But if such a situation arises, then the person may experience more intense side effects or allergic reactions.
It is noteworthy that there is no antidote, so doctors prefer symptomatic treatment of an overdose.
How to treat children with Contrikal
The fact is that the drug in question can be prescribed even in infancy. The most important thing is to follow the exact dosages prescribed by your doctor.
In the field of pediatrics, they know that the maximum dose of contrikal per day is 14 thousand units per 1 kilogram of body weight.
If the child weighs more than 5 kilograms, then the treatment is carried out with dosages for adults.
How Contrikal interacts with other medicines
The drug in question is often given concurrently with another drug.
This suggests that a specialist must take into account several nuances of their interaction:
- Contrikal should not be administered with a beta-lactam antibiotic and solutions containing lipids, amino acids, and dextran.
- The agent in question can suppress the effectiveness of urokinase or streptokinase.
- If you introduce Contrikal and heparinized blood at the same time, then it can clot longer.
In order to prevent complications, you should consider these points before using Contrikal.
Drug analogs
On the shelves of pharmacies there is a large selection of drugs - analogues and drugs - synonyms of the medication in question.
Preparations - synonyms - these are drugs, the pharmacological properties of which are similar to Contrikal, but they have different active substances. This includes Gordox, Ingiprol.
Preparations - analogues - drugs containing active components identical to Contrikal. They are as follows: Aprotex, Vero - Narkap.
If it is necessary to replace Contrikal, then the choice of the drug should be carried out exclusively by a qualified specialist.
The cost of the drug in question differs depending on which pharmacies it will be purchased from.
The final amount ranges from 600 to 900 rubles, which makes Kontrikal not available to all segments of the population.
