Pancreatitis symptoms and treatment: what are the causes

Pancreatitis is increasingly diagnosed in people of all ages, and the main reason is the failure of the outflow of enzymes that are necessary for normal digestion.

They are produced in the pancreas, and when it becomes inflamed, disorders begin.

There are several main reasons for the development of the disease, there are different symptoms that you need to know about in order to timely determine the disease and start treatment.

Features of the disease

Pancreatitis is an inflammation of the pancreas, when degenerative changes and processes occur in the organ.

When the disease appears, the secreted enzymes are not able to normally penetrate into the duodenum, therefore, a negative effect on the gland itself appears, it gradually destroys itself.

In fact, pancreatitis and its manifestations are self-digestion of tissues, when a chemical attack begins, healthy organ tissues are replaced by connective tissues.

The pancreas is considered a complex organ of the entire digestive system, which is almost impossible to restore when disorders and diseases appear.

The main functions of the gland are regulated by metabolic processes and the correct functioning of the gastrointestinal tract. Among the main tasks of the pancreas are:

  1. The production of enzymes that are necessary for the breakdown of food into useful substances, which are quickly absorbed by the body.
  2. The release of insulin and other hormones, which are responsible for storing nutrients, energy and using them when needed.

With organ inflammation, the symptoms and treatment are always different, it all depends on the cause, severity and type of disease.

Main reasons

Often, pancreatitis develops in people who do not monitor their diet and lifestyle, and often drink alcohol.

As statistics show, many people have an acute form of pancreatitis, which develops due to alcohol.

40% of alcohol abusers have pancreatitis. In only 5% of people, the pathology can be inherited.

There are the most common causes of the disease, among which are:

  1. Gallstone disease. Approximately 25% of cases of pancreatitis develop due to impaired functioning of the gallbladder and its ducts. Both organs of the dough are connected with each other, therefore, with inflammation of the gallbladder, an inflammatory process often develops in the pancreas. In addition, gallstones can occlude the pancreas, resulting in enzyme deficiency.
  2. Alcohol abuse. Approximately 50% of patients with pancreatitis previously drank alcohol in unlimited quantities. Due to the large amount of ethyl alcohol in the blood, an excess of enzymes begins, which slows down in movement and can be activated without entering the intestines.
  3. Wrong food. If a person eats fatty foods, often uses spicy and other types of spices in the diet, as well as heavy and unhealthy foods, then the likelihood of developing pancreatitis increases significantly. In addition to food, the disease can appear when the meal regimen is violated, if the intake is plentiful or a long time passes between meals. A very dangerous condition can be a case when fatty foods are consumed and alcohol is immediately taken.
  4. Internal organ damage or postoperative period. Such problems can lead to acute inflammatory processes, often cause additional infections and complications.
  5. Infectious diseases and inflammation of other organs of the gastrointestinal tract. Often pancreatitis develops due to viral hepatitis, and may appear due to some colds.
  6. Genetic predisposition. If the family has relatives with a chronic form of pancreatitis, then the disease can be transmitted to the child at the gene level. The first symptoms may develop immediately after birth or at an early age.
  7. Poisoning. When the body is poisoned with food or chemicals, an increased release of active enzymes begins, which have a negative effect on the gland itself, causing its inflammation.

You can understand that inflammation begins by the characteristic symptoms that every person should know. This will allow you to timely suspect something was wrong and start the correct treatment.

Types of pancreatitis

In medicine, the disease is classified into 2 main types: acute and chronic. In acute pancreatitis, severe inflammation begins, which has pronounced symptoms.

Most often, this type appears for several provoking factors:

  1. Frequent and prolonged use of alcohol. As a rule, the problem appears in men.
  2. Gallstone disease, which is more often diagnosed in women.
  3. Injury and surgery to the abdomen or stomach.

Within the limits of acute pancreatitis, there are several main phases of the course of the disease, including:

  1. Enzymatic, its duration is 3-5 days.
  2. Reactive, which lasts up to 2 weeks.
  3. Sequestration, appears from 2 weeks of the course of the disease.
  4. The outcome phase can be from 6 months or longer from the very onset of the manifestation of the disease.

Chronic pancreatitis is a disease when the inflammatory process has mild symptoms, and the development of the disease itself is slow.

The main work of the pancreas is gradual failures. The result of this form can be the replacement of healthy tissues with connective tissues.

The chronic form has several subspecies, which include:

  1. Toxic-metabolic.
  2. Idiopathic.
  3. Genetic.
  4. Autoimmune.
  5. Recurrent.
  6. Obstructive.

Chronic pancreatitis can be primary, when the disease develops on its own, or secondary - it appears due to other diseases of the gastrointestinal tract.

First symptoms

With the development of an acute form of pancreatitis in a person, the state of health and general well-being begins to deteriorate sharply.

At this time, you need to provide immediate assistance, consult a doctor yourself or call an ambulance.

In the acute phase of the disease, a sharp pain appears, which begins under the ribs on the left or right, and can radiate to the lower back, back and other parts of the body.

By nature, the symptom will be shingles, and the lesion of the gland is large-scale, perhaps even inflammation of the entire organ.

In addition to pain, vomiting begins, and bile particles can be seen in the mass. After vomiting, relief does not occur, there may be other dyspeptic disorders.

Similar manifestations will be with an exacerbation of the chronic form of pancreatitis, but the localization and intensity of pain depends on the degree of damage to the gland.

The main symptoms

The main signs of pancreatitis begin to appear due to the impossibility of normal excretion of enzymes that the organ secretes.

They do not enter the duodenum, and their activation begins in the gland itself. This condition causes inflammation and gradual destruction of the organ, enzymes themselves destroy the tissue of the pancreas.

In the acute form of the disease, patients will have the following symptoms:

  1. Pancreatic colic. Severe pains under the ribs on the left, which radiate to the back. During the first 3 days, the symptoms will be shingles. After a while, the strength of the pain subsides, becomes dull and lasts for a week.
  2. Vomit. In the acute form of the disease, the symptom is repeated very often, the vomiting itself is painful and after it the condition does not improve. Such a symptom often appears at the very beginning of pancreatitis and is complemented by abdominal pain, but the development of the disorder in later stages, for example, in the chronic phase, is not excluded.
  3. Nausea and general disturbance of the digestive system.
  4. Bloating. The symptom appears in the upper part of the abdominal cavity, and on palpation, severe pain may develop, especially if the abdomen is strongly pressed. While listening, the doctor may hear noises clearly.
  5. Temperature rise. The more the organ is affected and the more significant the inflammation, the higher the indicator on the thermometer.
  6. Dry mouth.
  7. Plaque of white on the tongue.
  8. Pain in the head, increased sweating.
  9. Skin discoloration; it can be pale, bluish, or yellow. This symptom appears not only on the skin, but also on the mucous membrane. Cyanosis is the least common, but such a sign indicates a severe course of the disease in an acute or exacerbated form. Most often, the blue color will be in front of the abdomen, on the sides and around the navel, a symptom may appear on the face.
  10. Increased pressure, frequent work of the heart.
  11. Breathing disorder. The patient begins to breathe quickly and shallowly, the patient himself thinks that there is not enough air. Shortness of breath develops due to disturbances in water and electrolyte balance caused by frequent vomiting. At this time, sticky, cold sweat appears.

The described symptoms worsen very quickly, so you need to quickly seek help and stop the acute condition.

It is best, if acute pancreatitis is suspected, to call an ambulance and try not to use any measures on your own, especially not to take medications so as not to smear the clinical picture.

In the chronic form, the symptoms are slightly different, their intensity is much lower, and among the main signs are:

  1. Abdominal pain that is temporary or permanent. As a rule, pain syndrome begins with improperly selected dishes and products during a meal or drinking alcohol. The pain will be in the upper abdomen, often radiating to the back.
  2. Flatulence and bloating.
  3. Nausea and vomiting.
  4. Decreased appetite.
  5. Stool disorder, with chronic pancreatitis, persistent diarrhea appears.
  6. Dehydration due to diarrhea and vomiting. This problem causes weight loss.
  7. The appearance of a yellow tinge on the oral mucosa.
  8. The development of diabetes is characteristic of the severe and prolonged course of chronic pancreatitis.

If the pain is mild during an exacerbation, then they can be several months. Patients may feel depressed, weak in the body, and decreased appetite.

Possible complications

Pancreatitis can be dangerous because it increases the likelihood of complications.

Among them, the following types can be distinguished:

  1. Pancreatic abscess.
  2. Dying off of organ tissue.
  3. The appearance of a false cyst.
  4. Ascites.
  5. Complications on the lungs.
  6. The development of diabetes.
  7. Blood poisoning.
  8. Peritonitis.

The last two complications are often fatal. When the described problems appear, a change in the course of the disease begins.

The place and nature of the pain changes, as well as the strength of other symptoms.

Diagnosis

Diagnosis and definition of the disease should be carried out as early as possible, which will reduce the time for treatment and restoration of basic functions.

In addition, early diagnosis improves overall prognosis and excludes the development of complications.

The best way to determine the degree of damage to the pancreas and the general course of pancreatitis is a comprehensive examination.

After a visit to the doctor, the procedure is carried out in several main stages:

  1. An overall assessment of symptoms is carried out, complaints are collected, examination and palpation are done.
  2. Blood, feces and urine tests are collected for research.
  3. Enzymatic tests are done.
  4. Use instrumental diagnostic methods.

Several methods apply to the last point:

  1. Ultrasound of the abdominal cavity to assess the condition of the pancreas, its size and the presence of calculi in the canals.
  2. X-ray allows you to see the state of the intestine, atelectasis and other possible disorders.
  3. Gastroduodenoscopy.
  4. Angiography, which is performed to assess the state of the vessels.

If the results are used exclusively by ultrasound, then the diagnosis may not be accurate. Often, with pancreatitis, the symptoms do not have a pronounced picture or are hidden under the signs of other diseases.

Methods for diagnosing patients may differ from those described, so doctors adjust the technique for a particular patient.

Only after receiving the necessary information is the method of further treatment determined.

Treatment

In acute pancreatitis, therapy is carried out in a hospital setting, so that patients are under constant supervision of doctors.

This excludes the development of serious complications and disorders. In the chronic form, treatment can be carried out at home.

To do this, you need to adhere to proper nutrition, lifestyle. Perhaps use some medications. As soon as symptoms of exacerbations of pancreatitis appear, an ambulance should be called immediately.

First aid

If an attack of illness occurs, then before the arrival of the team of doctors, the patient needs to be helped. For this, maximum rest is used.

It is possible to reduce the intensity of pain and other disorders by adopting a sitting position and tilting the body towards the legs.

It is forbidden to eat any food, you can only drink 50-70 ml of alkaline water without gas every half hour.

In the acute course of the pathology, a person will need to fast for several days in order to relieve the load on the pancreas.

During an attack with severe pain, apply cold to the abdomen, this helps to eliminate inflammation and swelling of the tissues. Shallow breathing can be used for relief.

It is forbidden to take pain pills or medications for inflammation until the doctors arrive.

This significantly complicates the diagnosis and determination of the causes of exacerbations. If the pain cannot be sustained, then antispasmodic drugs are allowed, among which No-shpu or Drotaverin are distinguished.

These medications can be used in further treatment to reduce spasms and inflammation.

In a hospital setting, doctors apply intensive conservative treatment. All drugs are injected into a muscle or vein; pills are often not used during hospitalization.

To eliminate the symptoms of intoxication, pain, saline solutions, protein agents and glucose are used.

Medication

With pancreatitis, conservative treatment is always used, this is one of the first measures to improve the condition. Possible drugs include:

  1. Solutions that restore water-salt balance. To do this, you need to drink a sodium chloride solution. It is recommended to use the product in case of profuse and prolonged vomiting.
  2. Enzymatic drugs are prescribed to stabilize the digestive tract. They are necessary in case of enzyme deficiency, and treatment is carried out with the help of Creon, Pancreatin, Festal tablets and their analogues.
  3. Vitamin complexes must be used, which must be used for chronic pancreatitis.
  4. Parenteral nutrition is used in case of insufficient absorption of nutrients by the intestines, as well as a strong violation of the digestive system. To do this, use droppers or the introduction of drugs into a vein. Amino acids (Alvezin, Aminosol) and electrolyte solution are often used.

Conservative therapy does not always give positive results. If it does not appear to be effective, medicines can be replaced or supplemented with other drugs.

If after this there is no improvement, doctors prescribe surgical treatment.

Operative treatment

Operations are indicated for severe inflammation not only of the pancreas, but also of the entire abdominal cavity.

Surgery is also performed with increased jaundice, disruption of the gallbladder, or the appearance of purulent inflammation.

The operation can be emergency or planned. In the first case, it is done at the very beginning of the development of pancreatitis, the intervention is carried out urgently, and the main indication is the development of peritonitis, which appears due to the acute course of the disease or exposure enzymes.

Elective surgery is indicated if acute symptoms in the gland disappear, but conservative remedies do not improve.

Before it is carried out, the patient undergoes a detailed diagnosis, which excludes complications and relapses.

Food

For the effectiveness of the treatment of pancreatitis, enhancing the action of medications, the patient must strictly adhere to the rules of nutrition.

For this, a diet No. 5p according to Pevzner has been created, which is prescribed for inflammation of the pancreas.

The patient's diet should be adjusted so that the consumption of carbohydrate and fatty foods is reduced in it, but protein foods are increased.

You need to eat fractionally, often, the approximate number of meals per day is 5-6, and the break between them is 2-3 hours.

In the acute course of the disease, complete fasting should be used for 2-3 days, at this time you can use only mineral water without gas or a rosehip decoction.

After eliminating acute symptoms, the patient needs to adhere to the basic rules of the diet:

  1. Eat healthy and healthy food, which will contain lipids, proteins, carbohydrates.
  2. It is recommended to use vitamins that can dissolve fats.
  3. Completely refuse products that contain preservatives, chemicals and other substances that can irritate the mucous membrane.
  4. Do not use fatty, smoked and spicy foods, give up spices.
  5. Use correct cooking methods. All products must be chopped, grinded, and it is better to steam, boil or bake.
  6. The temperature of the dishes should be around 25-50 degrees.

When the acute signs of pancreatitis are completely stopped. Light soups with 2-3 broth, dietary meat and fish, low-fat kefir and cottage cheese, as well as other products are added to the diet.

Among the main dishes and products that are allowed for consumption include:

  1. First meal. They need to be made on the basis of cereals, vegetables or chicken broth. In the latter case, 3 decoction is used, from which all the fat is removed, and it must be prepared on the basis of chicken fillet.
  2. Second courses. It is recommended to use no more than 1 egg for an omelet; you can cook dietary meat or fish.
  3. Cereal dishes. This includes almost all pasta, rice, buckwheat, oatmeal, and 1-day bread or crackers.
  4. Dairy. It is allowed to use only low-fat types, including milk, cottage cheese, yogurt and kefir. During remission of pancreatitis, you can add a little butter to dishes.
  5. Plant food. Fruits can be used only sweet, it is advisable not to use exotic. Tomatoes are not allowed from vegetables. All plant foods are best baked, boiled, or steamed. Raw foods are not always available.
  6. Sweets. Only honey and homemade jams are allowed, but such products are used in minimal quantities.
  7. The drinks. Mineral water without gas, fresh vegetable juices, rosehip decoction, herbal teas are allowed.

What you can and cannot eat should be told by the attending doctor. The list of products for chronic and acute pancreatitis may differ, it is selected for each patient individually.

Folk remedies

Folk remedies can alleviate the condition of patients, relieve symptoms. They are not used as stand-alone therapy, but are a good adjunct to medical treatment.

Among the possible recipes for pancreatitis are:

  1. Golden mustache. Add 0.7 liters of water to 1 large chopped leaf, cook for 20 minutes, leave for 2-3 hours and drink 25 ml in a heated form before meals.
  2. Mint and honey. The combination is used to replace ordinary tea, the drink will have a choleretic effect, improve the yield of enzymes.
  3. Propolis. The crushed plant should be thoroughly chewed between meals to reduce pain and improve the functioning of the digestive system.
  4. Rowan. Berries can be used for treatment and prevention, they can be eaten fresh by handfuls or made as teas.
  5. Oats. Add 1 liter of water to a glass of cereals and leave to brew for 12 hours, cook for 30 minutes, leave for another 12 hours. After filtration, add water to 1 liter and take 100 ml 30 minutes before a meal, with a course of 1 month.
  6. Rose hip. With chronic pancreatitis, the broth can be used constantly, instead of regular tea. The daily rate is 400 ml. For cooking, you need 1 tbsp. add 250 ml of boiling water.

The described and other traditional medicine can be used after prior consultation with a doctor. Although they are natural, they can cause undesirable consequences.

Prophylaxis

It is possible to exclude relapses of the disease, as well as reduce the likelihood of developing pancreatitis, if simple preventive measures are followed.

These include:

  1. Elimination of provoking factors: rejection of alcohol and other bad habits, normalization of metabolic processes.
  2. Constant adherence to proper nutrition. We must try to exclude harmful and heavy foods from the diet. Enrich the menu with herbal products, vitamins.
  3. To monitor the state of the digestive system, with advanced diseases in a chronic form, it is necessary to adjust the treatment. Patients should visit a doctor more often in order to exclude exacerbations, and in case of relapses, immediately seek help.

Knowing the symptoms and treatment of pancreatitis, the main causes and types of the disease, you should monitor your health.

Timely determine the pathology, which will allow for treatment in the early stages, when there are no complications and significant degenerative processes.

Treatment of mild pancreatitis at the beginning of development, allows you to completely cure the disease, preventing its transition to the chronic phase.
