The oniroid is an inadequate state, manifested by a deep obscuration of awareness, while the person behaves rather ambiguously. Double orientation, the state of the matrix - is a characteristic terminology used for this type of pathological productive mute consciousness. Oneyroids are a form of productive obscuration, because a person experiencing these states has the ability to contemplate unknown things that no one else notices. It is important not to contradict these people, especially in their sleepy experiences, to prevent their aggressiveness. In this state, the person's hallucinations, intertwined with reality, turn into a scale scene, sometimes even historical events.

Oniroid - what is it?

A oneiride is a common psychiatric word derived from the Greek language and meaning a dreamlike sight. This concept, slightly modified, was first described at the end of the 19th century by the psychiatrist Regis in intoxication. And for the first time, the onyroid was named as it is said up to now, at the beginning of the 20th century and it was done, by the outstanding Clérambo, who had a hand in describing the many states in the psychiatric industry. This term has been further developed after a long time, it took many scientists - physicians and researchers - to study it and describe it thoroughly. And it does not surprise, the state is not too widespread in comparison with many others.

The onyroid syndrome is a syndrome of confusion of consciousness, related to productive syndromes of disturbance of awareness. In the onyroid there arises an uncontrolled influx of periodically appearing fantastic dreamlike delusions of hallucinations in the form of paintings completed in their essence, which alternate in a separate, purposeful sequence and create a single whole.

Onyeroid is accompanied by a partial, and sometimes full alienation from the surrounding world. There are violations of self-awareness, accompanied by various affects, the most frequent - manic or depressive. There are signs of catatonia with the preservation in the consciousness of the composition of experiences under amnesia to reality.

Much in psychiatry, as in all medicine, is changing radically. So the term "onyroid" came into the subtype of delirium, therefore in ICD 10, as well as, sometimes, in life it is difficult to separate them. But nevertheless, these conditions have their own distinctive differences, which are well known to practicing psychiatrists.

In the domestic literature use the classification of onyeroid for Snezhnevsky, which has several subclasses. An important criterion is orientation, so there is a classification by such a criterion. The dreamlike onyroid resembles a dream-event, while the person is alienated from the environment, but the events of his dreams attract him very much. The fantastically illusory onyroid demonstrates a different state, manifested in mixed orienteering, while both reality and fantasy experiences are present.

According to the reasons, onyeroids, like many other psychiatric problems, there may be many, but a few very significant and very different, worth highlighting, namely:

• Alcoholic anoyoid is a separate, nothing like kind of onyroid which progresses fromdelirium for the same reason. It differs from the usual onyroid, because alcoholics have a completely different perception.

• Oneroid after the stroke is of an organic nature, often after such severe conditions, this complication is possible. Often, in contact with such a neurological inpatient, this state leads to fear.

• Since the occurrence of affect is very characteristic of an onyroid, in the form of a special behavioral mood, this classification is also very relevant. Behaviors with different affects are very different, which affects the course of the process and the treatment used.

Causes of Oniroid

Oneiroid is a nonspecific pathological picture, that is, there is no one characteristic disease that causes its occurrence. The causes of this pathology, as well as all subspecies of the obscuration of consciousness are many. But the main division of this pathology is still there. The endogenous nature of the diseases that cause the aneuroid are typical psychiatric pathologies. Schizophrenia can be accompanied by such conditions, and its onyroid-catatonic form is not diagnosed if there were no such conditions. Very rarely, with the so-called psychotic form of bipolar disorder, a state can appear as an onyroid, then there is a pronounced affect, manic or more often vice versa.

If an onyroid has an external cause that does not emanate from the brain itself, then such a cause has the name exogenous. All intoxicating subspecies, do not depend on the chemical nature of the substance. This condition can provoke poisoning with chemicals, substance abuse using stationery, gasoline, solvents. Addiction is also capable of leading to such consequences.

Alcoholic anoyoid appears as an aggravation of delirium, in a person suffering from alcoholism. Its occurrence is directly related to stopping the intake of alcohol for a certain period after long drinking-bouts. Not only after alcohol cancellation is possible such a condition, but also when using substandard substitutes for alcohol and poisoning them.

In medicine, the condition of the onyroid can also arise, not only in diseases, but also in the arrest of some. With surgical pathology, anesthesia is used that can cause confusion, for example, onyroid. A lot of viral diseases especially in children that lead to intoxication and the subsequent development of this pathological condition are: influenza, acute respiratory viral infection, chicken pox, EBV, infectious mononucleosis, CMV.Bacterial diseases with fever are also accompanied by an onyroid, especially massive septic complications. In the case of direct damage to the brain tissue, symptoms from psychiatry are unavoidable, which is why an oniroid is a companion of meningoencephalitis of virtually any etiology.

Epilepsy, as a neurological disease, manifests itself not only in the form of usual convulsive seizures, but also in the form of their equivalents. With a long course of the disease, psychoses arise, and the onyroid as a subspecies of obscuration of consciousness is also possible, but much less often than twilight.

Non-infectious diseases also often provoke obscurations of consciousness in the form of onyeroid. Especially affects the body with expressive violations of material exchange and vascular damage.

Oniroid after stroke occurs in severe forms of this disease with severe vascular lesions. Myocardial infarction and all types of deficiency in the body are also capable of provoking an onyroid. Rheumatological disorders, especially vasculitis and lupus easily provoke onyroid conditions due to disruption of vascular work. The lack of individual vitamins or complexes, for example, neglected pellagra, due to a disturbance in the metabolism of brain mediators provokes an onyroid. Endocrinological problems and neoplasia are also provocateurs of oniroid, due to their metabolic disturbances and powerful intoxication mechanisms.

Symptoms and signs of onyroid

Oneyroidnoe obscuration of consciousness in its symptoms is similar to all types of obscuration of consciousness, but it has some difference. It is important to take into account the emotional component of this pathology, since the effect is very pronounced. When manic, the person is completely captured by his experiences, is active, energetic, and with depressive wilted and sad, but everything also, residing in his experiences. Their speech is rather slow, incoherent and does not correspond to situational aspects. Thinking is broken, orientation is dual, the sequence is broken. Amnesia is incomplete and manifests itself as a memory disorder with pronounced memories of pathological experiences, but the forgetfulness of real events. The effector-volitional sphere is also affected, instincts are lowered, emotions are inexpressive, the person is confused and drowsy.

The first to form violations of emotions, a person is sluggish, bezynitsiativny, as if something upset. But this mood, like any other, is changing unsteadily and dramatically for another reason. Often such people are afraid of sleeping, because their dreams are awesome, terrifying, they try to avoid such terrible moments to the maximum. Insomnia is a frequent companion of consciousness disorders. The signs of affect that can not be concealed are impossible, the person is touched or in utter despair, as it seems to him.

When talking with a person, immediately there will be noticeable behavioral shifts that will manifest themselves in delusions. Perceptual disorders are also not uncommon in these states, and there may be illusions and scenic hallucinations.

The dreamlike onyroid is especially evident in the presence of hypnagogic hallucination, while the person, closing his eyes, sees certain scenes, people. When communicating with such patients, there will be no sense, they say pieces, broken words, it is impossible to trace the thread of conversation. The speech itself is unintelligible, changing, then accelerating, then slowing down. The orientation in itself is partially preserved, but still it is impossible to keep in touch with the person, he is confused, speaks rather to himself than with someone else, and even indistinctly. In time, as well as the place around, they are absolutely not oriented, time for them stretches or runs. At the same time, people are confident in the artificiality of their actions, there are psychic automatisms.

The universe of such a person is drastically changing, while the person commits the usual actions, but his consciousness lives absolutely his own life, unlike anything else. At the same time, he does daily affairs, simultaneously fights against evil, destroys the devil or cosmic evil.

The facial expressions are not thematic, the face can even be amylic. Over time, this duality is exacerbated, until the complete departure into the world of dreams. Movements become similar to each other, it already so falls into its dreamy state that it can not notice real dangers.

It is also important to distinguish the underlying cause of the disease, since the symptomatology is different. For infectious reasons, there is intoxication and complaints from the side of the head. When poisoning there are additional complaints, by the type of diarrhea, vomiting. When using drugs, it is important to find an appropriate medical history.

Stages of development and stages of oniroid

The developmental stage of onyeroid has very blurred outlines, but still the classical onyroid is divided into separate stages, smoothly flowing from each other.

Since the beginning of the onyroid development, typical vegetation, high sweating, reddening of the face, pupils are enlarged. On the part of the body can also appear different disorders, problems with appetite, insomnia, minor unpleasant problems at the level of the body. After this, the person is formed delusional ideas, which are actively introduced into the clinic of the disease. Their meaning greatly influences the behavior of the individual and the clinic of the disease itself. They can be paranoid, or even Manichaean, on religious subjects, when a person seems to be sitting on the one side with an angel, and with another demon, and argue over his subsequent fate. After that, there is already a dual orientation in the form of affective - delusional disorders. The person is absolutely disoriented, derealized, she feels a special connection, understands that she is in a different dimension. At the same time, she can realize herself at the same time as someone else and herself, God, for example, and this is already the progression of depersonalization. This symptomatology is characteristic of Alice's syndrome in Wonderland, which is not excluded for onyeroid. After the personality regresses even more expressively and manifests already in the form of a complete fantasy alienation from the outside world and experiencing a multitude of fantasy, illusory, hallucinatory visions. The last most threatening stage is a catatonic onyroid, in which catatonia is included, according to the type of wax flexibility. Often there is a symptom of an air cushion, when a person keeps his head on weight.

In domestic psychiatry periodicals are more expressive, but also has five stages:

- At the initial stage, depending on the etiology, there are different prodromal first manifestations. And this stage lasts no more than a month, but is usually shorter.

- At the stage of crazy mood, according to the name, nonsense crystallizes, delusions are formed. A person starts to express his guesses, the duration of this stage is up to several days, but more often hours.

- This is followed by the third stage with the already crystallized delirium of dramatization, when patients suspect that for them all play roles, values, when they realize that everyone is looking at them in a special way, and the intermetamorphoses in which they themselves and the whole world around radically change. It lasts for weeks, but more often days.

- Then there is acute paraphrenia, in which patients elevate themselves, they are completely absorbed by a variety of illusions.

- At the stage of the last, true onyroid, all the symptomatology already fully enters into action and reaches its apogee.

Depending on the reasons, the stage can develop in different ways, the most classical oneroid is observed in schizophrenics, it has the term endogenous, and for organic reasons, there are usually seizures, but the final stage is similar in all species. Passes an onyeroid with the reverse development of symptoms.

Treatment of oniroid

The ONEroid condition is quite dangerous, a person may not even get out of it. To hospitalize in a psychiatric ward such a person must be urgently, and there already begin a phased treatment. First, symptomatic remedies are used to withdraw from a formidable condition, and then we are looking for and treating the cause.

Primarily used neuroleptics: Azaleptin 25 mg, Eglonil 25 ml, Carbidine, Triftazin, Haloperidol, Metaterin, Frenolone, Etaperizin, Mazeptil, Trisedil, Aminosin up to 1100 mg, Propazinum, Tizercin up to 290 mg. Electroconvulsive therapy is very well influenced, but in the domestic sciences, it is not very developed.

Nootropics that affect affective-effector disorders are also actively used. Aminalon, Piriditop, Acephan, Pantogam. The pyriditope, which proved to be a good antidepressant and sedative, is used for depressive affect.

Alcoholic aneuroid is treated like delirium. Transillifying agents for relief of insomnia and agitation: Sibazon 3 ml, Relanium in a similar dosage. Means with a hypnotic effect - Sodium oxybutyrate up to 28 ml, Ridazin up to 340 mg. Excretion of toxins is carried out with Cytoflavin up to 9 ml or L-lysine, Sodium thiosulfate, Atoxyl 12 mg, Thiogam 560 mg. Less commonly used Korglikon, Soda solution, Strofantin, Cordiamin, Papaverin, Platyfilin, Polyglukin, Dibazol.

To prevent swelling of the brain tissue, which is not uncommon for an alcoholic, Mannit, Lasix, is used.

Oniroid after a stroke is stopped by neuroleptics, and then all kinds of vascular drugs are used: Troxovasin, Mema, Ginkgo biloba, Bilobil.

A dreamlike onyroid is stopped with Clopixol-Acuphase up to 12 mg and Fluansol Depot up to 19 mg.
