Disease of Itenko-Cushing
Disease of Itenko-Cushing is a serious, chronic disease of the brain, which is caused by the defeat of the hypothalamic-pituitary region. The disease can occur after cranio - brain injury, neuroinfections due to pituitary tumors
Symptoms
- Person becomes "lunoobraznym"
- There is an increased deposition of fat in the areas of: belly, shoulders, neck and
face - As a result of degeneration of muscle tissue occurs thinninglimbs
- increased blood pressure( hypertension)
- Reduced bone density( osteoporosis)
- Increased blood sugar( hyperglycemia)
- muscle weakness appears
- appearstsya clearly expressed venous pattern on the limbs and chest
- Women are possible manifestations of male pattern hair distribution on
man's disease Cushing's gradually becomes quite specific appearance - lunate shaped red face, fat body with thin limbs. On the stomach because of obesity, stretch marks appear. Places of friction( elbows and neck) and skin folds due to increased pigmentation become darker than the rest of the skin. The disease significantly reduces immunity, so that about 50% of patients die from infectious diseases. Against the backdrop of the Itenko-Cushing disease, serious psychic abnormalities often develop.
There are three forms of the disease: mild, moderate and severe form.
The disease can be progressive( symptoms developed over six to twelve months) and torpid( gradual increase in symptoms within three to ten years)
Treatment
Until recently, X-ray irradiation mezhutochko-pituitary region was the only treatment for this disease. At present, the physician's arsenal are more modern and advanced means and methods of treatment, which is fashionable to divide into two groups:
- Methods based on the local suppression of hyperfunction of cortex
adrenal - Methods to eliminate focus in the hypothalamic-pituitary region
EvenIn spite of the fact that in our time the treatment has become much more effective - quality therapy of the Itenko-Cushing's disease in children has not yet been fully developed.
Drug treatment
This type of treatment is based on the use of drugs that inhibit the secretion of adrenal cortex hormones( mamomite, chloridant).Depending on the severity of any symptoms additionally prescribed protivodiabetic, anabolic and antihypertensive drugs
Surgical treatment
This type of treatment is divided into two types: adrenomectomy and transsphenoidal adenomectomy.
Adrenomectomy is used as an additional method of treatment for radiation therapy and consists in removing one( or both) of the adrenal gland.
Transsphenoidal adenomectomy consists in the removal of the pituitary tumor. In the case of a successfully performed operation, the first effect manifests itself within ten days and is expressed in a decrease in the main symptoms of hypercorticism, a decrease in appetite and overall improvement in well-being.
Radiation therapy
By means of radioadenomectomy( proton beam), pituitary activity is suppressed. The effectiveness of radiotherapy is manifested in 6-12 months and is conditioned( in the majority of patients) by a complete normalization of clinical and hormonal parameters.
