Pancreatic insufficiency: symptoms and enzyme

The pancreas is considered to be the main organ that stimulates the digestive process.

Insufficiency of the pancreas is a disease that is caused by a violation of the production of hormones necessary for the proper functioning of the body.

This pathology is caused by the destruction of normal tissues and cells inside the organ and their gradual replacement with scar tissue.

In the future, this leads to a deterioration in the functioning of the gland in the production of important enzymes and hormones.

As a result of this, disturbances in the work of all internal organs can occur. Therefore, it is extremely important to start therapy for the disease in a timely manner.

Causes

The organ produces special enzymes inside the body, without which the process of food processing is impossible.

In some cases, the proper functioning of the pancreas will be disrupted and the amount of components produced will decrease.

Such pathological disorders are called pancreatic insufficiency. The main provoking factors in the formation of pathology will be:

  • degenerative processes in the organ;
  • vitamin deficiency (lack of vitamins B, C, E, PP, nicotinic acid), which causes liver diseases and the appearance of gallstone disease;
  • decreased protein content and anemia;
  • violations in the diet - a large amount of fatty, spicy in the menu, excessive consumption of alcoholic beverages;
  • viral pathologies of the stomach, pancreas, duodenum;
  • exacerbation or chronic form of pancreatitis;
  • helminthiasis;
  • prolonged use of medications;
  • improper fasting;
  • impaired metabolism;
  • malfunctions in the functioning of the small and duodenum, degenerative processes of the intestinal microflora;
  • congenital malformations of the pancreas.

In addition to these factors that provoke the formation of disorders, there are also many reasons that affect the appearance of disorders in the gland.

In certain situations, heredity plays an important role in the appearance of malfunctions in the gland.

In some cases, even with an active lifestyle and a balanced diet, disorders in the gland appear. Such deviations in the work of the organ in question can provoke various pathologies.

Symptoms

It must be said that in case of pancreatic insufficiency, the symptoms of the pathological process appear sharply and after a certain period of time (by analogy with the chronic form of pancreatitis).

One of the main manifestations that characterize a lack of enzymes is stool disorders, becoming liquid, plentiful, with a greasy oily consistency, a gray tint and a pungent aroma rot.

It also contains fragments of unprocessed food.

In addition, such a pathological process is accompanied by:

  • general malaise and decreased physical activity;
  • progressive weight loss;
  • nausea;
  • constant gag reflex;
  • loss of appetite;
  • feeling of a full stomach;
  • heartburn;
  • gas formation;
  • overflowing pain in the abdominal cavity.

Given the likely latent nature of the deficiency, it is extremely important if the above manifestations occur without delay to contact a specialist for diagnostics, making an appropriate diagnosis, taking preventive measures, as well as treating pathological process.

Diagnostics

Since the symptoms of pancreatic insufficiency, which are expressed by upset gastric motility (for example, nausea or gag reflex), in certain situations coincides with the symptoms of exacerbation of pancreatitis, for correct diagnosis, the implementation of certain clinical and diagnostic measures.

These include:

  • blood tests (general, biochemical, sugar);
  • analysis of urine for enzymes;
  • analysis of feces (general and for elastase);
  • Ultrasound;
  • CT and MRI;
  • endoscopy;
  • special testing.

The specific probe secretin-cholecystokinin test is the most accurate method for detecting deficiency.

Types of insufficiency

To date, there are 4 types of functional insufficiency of the gland.

Each of them is characterized by its own characteristics, from provoking factors of occurrence to symptoms and therapy. Highlighted:

  • Exocrine;
  • Exocrine;
  • Enzyme;
  • Endocrine.

As mentioned earlier, each has its own characteristics. Therefore, in order to fully understand whether there are differences and there are similarities in symptoms, you need to learn more about each.

To establish a specific type of pancreatic insufficiency, you need to know the recommendations a specialist who, starting from the manifestations, makes the correct diagnosis and prescribes an effective therapy.

Exocrine disorders

Exocrine insufficiency is characterized by the lack of the required amount of gastric juices that help to process food products. If such a disorder occurs, the following signs will appear:

  • Feeling of heaviness inside the stomach after eating.
  • Nausea.
  • Excessive gas formation and bloating.
  • Food products are partially processed.
  • Disorders with emptying (constipation or diarrhea).

However, similar symptoms can also appear in the presence of other pathologies. Therefore, to make a diagnosis, the patient needs to pass a large number of tests.

In patients with a similar pathological process, diabetes is formed, therefore, a blood test for sugar content is taken by them quite often.

When, after the diagnosis, a person is diagnosed with exocrine pancreatic insufficiency, the specialist will prescribe the appropriate therapy.

At first, therapy will involve the elimination of the factor that led to the disease.

When the production of pancreatic juice and enzymes is affected by excessive consumption of alcoholic beverages or dietary food, the patient needs to reconsider his lifestyle.

The diet for this type of disease is based on food that is rich in vitamins A, E, C. It directly has a positive effect on the pancreas.

Often, enzymes prescribed by a specialist are used to normalize processes.

Often the provoking factors for the appearance of such a pathological process will be dietary nutrition in order to losing weight with the rejection of certain types of products or complete starvation, excessive consumption of alcoholic drinks.

External secret failures

Sometimes there are cases when the native tissues of the gland are converted into fatty or scar tissue, as a result of which the pancreas no longer secretes the required amount of enzymes, since the required number of active cells.

This pathological process has a wider list of symptoms:

  • Painful sensations in bone tissues.
  • Loose stools, with fatty impurities.
  • Intolerance to spicy and fatty foods.
  • Frequent heartbeat.
  • Convulsive states.
  • Impaired blood clotting.
  • Shortness of breath.
  • Gas formation.

Therapy involves the following principles:

  • frequent and fractional meals;
  • the diet involves the exclusion of fatty foods from the menu, including fatty meats;
  • last meal no later than 7 pm and refusal of snacks at night;
  • alcoholic beverages are completely prohibited;
  • all permitted food products must be coordinated with a specialist supervising this process before final restoration.

Due to the fact that the body does not produce the required amount of enzymes and does not receive fats, carbohydrates are quickly removed from it.

They need to be replenished from healthy foods. Sweets are full of carbohydrates, but completely forbidden for consumption.

If the amount of plant foods on the menu has increased, the body will respond to this with bloating, which decreases with the use of bran.

The lack of enzymes is compensated by the use of Creon, Pancreatin, Mezim. The first indicator that the treatment is effective is the normalization of the stool and the disappearance of other negative symptoms.

Enzymatic disorders

The gastric juice contains enzymes that process various substances. Trypsin promotes the breakdown of proteins, amylase - fats, lactose - milk sugar, amylase and maltose - carbohydrates.

When one of the listed elements is not produced enough, pancreatic enzyme deficiency is formed. Main manifestations:

  • Loss or decreased appetite.
  • Weight loss.
  • Nausea.
  • Constant pain after eating.
  • Gas formation.
  • Constant diarrhea.

The pathological process under consideration is formed under the influence of prolonged use medications, the presence of hereditary or acquired pathology of the excretory duct pancreas.

Often, such disorders appear due to infection of the body with viruses.

The main principles of therapy are the elimination of those causes that caused disturbances in the normal functioning of the gland.

It is required to use medications that are prescribed by a doctor and follow a dietary diet.

In case of enzymatic insufficiency of the pancreas, therapy is necessarily prescribed by the treating person a specialist, starting from the diagnostic data and the lack of the secret of which enzymes will be found in specific situation.

Usually, a deficiency of pancreatic enzymes is treated with a complex method, combining enzyme replacement therapy and special dietary nutrition.

Substitution therapy is the mainstay of treatment, which consists in the use of special enzyme agents.

The goal will be to eliminate disturbances in the digestive processes and ensure the saturation of the body with nutrients and vitamins.

These enzyme agents include:

  • pancreatin preparations containing lipase, amylase and trypsin;
  • combined drugs, where bile components are added to pancreatin;
  • plant-based enzymes containing protease, lipase, fungal amylase, papain, absorbents and other components.

Endocrine disruptions

When the part of the gland that is responsible for the secretion of hormones is somehow affected, then endocrine pancreatic insufficiency will occur.

Under the influence of such changes in the work of the organ, a person develops a complex and incurable disease.

To determine the presence of organ damage, a specialist prescribes a blood test, as well as an ultrasound scan, CT scan. When the situation is extremely difficult, MRI is indispensable.

It is also necessary to pass urine analysis and make a coprogram.

The main manifestations by which it is possible to determine this type of disease are:

  • Regular urge to empty.
  • Loose stools.
  • Intense gas formation.
  • Dehydration.
  • Vomiting reflex.

When, at the end of the diagnosis, the suspicions have been confirmed, the specialist can prescribe a diet that excludes sweets in any form.

It takes some time to monitor the glucose content in the bloodstream. When the situation does not get better, it is necessary to connect the patient to insulin therapy.

Often, the pathological process under consideration manifests itself in an acute form of pancreatitis or exacerbation of a chronic one.

This is noted if the part of the gland that is responsible for the production of insulin becomes inflamed.

The phenomenon is called secondary endocrine insufficiency and basically disappears after the end of the therapeutic course of the disease.

Any disorders in the functioning of the pancreas threaten the patient's health, since the cells do not receive the required amount of useful components, they are not able to properly function.

Forecasts

Today, in fact, every 3 patient has some kind of gland insufficiency with varying degrees of severity.

Some randomly identified this phenomenon at the initial stage and were able to prevent the worsening of the condition, others discovered the pathology at the last stage, when the symptoms manifested themselves so strongly that they turned to doctors.

In any situation, everyone must follow all medical prescriptions.

When the patient follows a dietary diet and takes the prescribed enzyme substitutes, then in in most situations, he will be able to prevent the subsequent development of pathology and transformation into a more complex stage.

Insufficiency of the pancreas with a complete refusal of the patient from alcoholic beverages, fatty and spicy, observing all the prescriptions, has a high survival rate.

For example, when a patient, due to excessive consumption of alcoholic beverages, earned the considered pathological process, with a complete rejection of this addiction, he is able to live quite for a long time.

In any situation, the patient's health is in his hands, subject to the prescribed medical recommendations.
