Ulcerative colitis of the intestine: symptoms and treatment of ulcerative colitis

Chronic inflammation in the large intestine is called ulcerative colitis in medicine.

Such a disease is characterized by the development of ulcers on the mucous membrane that cannot heal, there are parts with dead tissue and bleeding wounds.

The pathological condition differs from the usual inflammatory process, since the mucous membrane becomes covered with ulcers, and with a prolonged course, the development of cancer is possible.

Description of the disease

Ulcerative colitis of the large intestine is an inflammation of the mucous membrane in the chronic phase, which appears due to genetic and external factors.

The disease can get worse, causing more severe symptoms. Pathology affects the rectum, after a while the inflammation spreads to other parts of the intestine. In medicine, this disease is called ulcerative colitis.

Most often, the disease develops in people 20-35 years old, as well as in old age after 60 years. Children are almost not exposed to such a pathology, but there are cases when colitis appears in a child.

In medical practice, only 15% of cases of the appearance of pathology in children have been recorded. Moreover, girls are most exposed to pathology in adolescence, and for younger children, colitis is characteristic of boys.

Ulcerative colitis of the intestine is divided into several types, its classification is as follows:

  1. According to the clinical course, the pathology can be typical, atypical, chronic.
  2. By localization, distal colitis (proctitis) is isolated, left-sided, which appears in the transverse colon, approximately to its middle. Also distinguish total with different types.
  3. By the severity of the symptoms.

Pathology changes the structure of the mucous membrane of the large intestine and in most patients inflammation, ulceration occurs at the end points of the intestine, namely the rectum and sigmoid.

Much less often in medicine, there are facts of damage to the entire intestine.

Causes

The exact cause of the disease still cannot be established by either scientists or doctors. In the formation of pathology, an important role is played by the autoimmune system, heredity, as well as certain external factors.

Intestinal colitis is most affected by people living in developed countries. Among the possible causes that lead to the disease, doctors distinguish the following:

  1. Intestinal infection of uncertain nature. But colitis itself is not transmitted from a sick person to a healthy person.
  2. An unbalanced diet, in which there are a lot of semi-finished products, fast food, and the menu itself does not have enough fiber and other useful substances.
  3. Genetic abnormalities.
  4. The use of medications, especially drugs for non-hormonal inflammation, as well as oral contraceptives.
  5. Frequent and severe stressful situations.
  6. Violation of the intestinal flora.

Ulcerative colitis of the large intestine leads to the development of pathogenic bacteria in the intestine, viruses develop, due to which the destruction of intestinal cells begins.

This process leads to the formation of ulcers. Some doctors believe there is protection against the development of colitis, namely:

  1. Removal of appendicitis, but only with its exacerbation.
  2. Smoking, the habit must be abandoned. If a person has not smoked, but has ulcerative colitis, after diagnosis he began to smoke, then the disease is aggravated much faster.

Under the influence of the described reasons, various signs of the disease appear, which are important to detect in a timely manner.

Symptoms

Ulcerative colitis of the intestine can proceed in the acute phase or in remission. The pathology itself manifests itself gradually, but develops rather quickly, after which the symptoms intensify.

The main signs of the disease, which can be from the digestive system:

  1. Abdominal pain in the form of contractions, which appear more often on the left side, is almost not relieved by medication.
  2. Diarrhea, the appearance of feces with particles of mucus, pus or blood. This symptom is noticed and gets worse in the morning or at night.
  3. Constipation can appear immediately after diarrhea, due to a strong spasm in the intestines.
  4. Bloating.
  5. False urge to bowel movements, which appear with constipation.
  6. Discharge of mucus or blood from the anus, regardless of the act of defecation

A small proportion of people may develop symptoms that are not related to disturbed bowel function and pathological processes in it.

Among them are:

  1. Joint pain and disease.
  2. A rash on the body or mucous membranes, especially in the mouth.
  3. Visual impairment.
  4. Malfunctions of the gallbladder, its channels and liver.
  5. Blood clots.

The described symptoms can be independent or appear together with intestinal disorders. The severity of signs outside the intestine depends on the strength of the inflammation.

If colitis is mild, then patients may experience discomfort and mild abdominal pain, which appears rather abruptly. Weakness develops in the body, the stool becomes 2-3 times a day, a liquid consistency, with particles of blood or mucus.

With a severe degree of the disease, the stool rises up to 8 times a day, and blood or draining is clearly visible in the feces.

Severe ulcerative colitis can be recognized by:

  1. Pain in the abdominal cavity that appears on the left. The sensations are sharp and strong.
  2. Weakness in the body.
  3. Feverish condition.
  4. Weight loss.
  5. Tachycardia.
  6. Pain in the region of the liver and bile.

With exacerbations of pathology, general signs of intoxication of the body appear.

It is possible to distinguish ulcerative colitis from catarrhal colitis by weight loss, often patients are emaciated and this is noticeable to others, their appetite disappears.

The described symptomatology can be stronger and weaker, it has a wavy nature.

If colitis is treated, remission begins, the characteristic signs recede, and relapses depend on the quality of treatment and further actions of the patient himself.

Complications and consequences

If you do not treat colitis, do not use the correct lifestyle, corrected nutrition, then the pathology leads to complications.

In this case, the main symptoms will become stronger, characteristic signs may appear:

  1. Strong, copious discharge of blood.
  2. Anemia.
  3. Formation of a megacolon.
  4. Peritonitis.
  5. Perforation.
  6. Inflammation of the joints.
  7. Violation and deformation of internal organs.

The consequences of the disease can be the most deplorable, therefore, with the development of the main symptoms of the disease, you should immediately contact a doctor for diagnostics and selection of a treatment regimen.

Diagnosis

Not all people, when pain and other disorders appear, immediately go to the doctor because of fear. They may be afraid of instrumental diagnostic methods or simply afraid to hear a terrible diagnosis.

Timely examination can save you from serious consequences and allow you to get rid of the signs of the disease.

The main methods for establishing the diagnosis and state of the intestine are divided into instrumental and laboratory ones. The first includes:

  1. Fibroilecolonoscopy is the main way to determine the condition. Examines the entire colon and lower small intestine. The technique determines the severity of colitis and its length; tissue can be taken for biopsy.
  2. Histological analysis is used to detect cancer cells and changes.
  3. Irrigoscopy - the essence is to conduct an X-ray using contrast agents, which indicate inflammatory processes, tumors.
  4. Hydro MRI - the study clarifies the state of the intestine, its tissues, excludes fistulas.
  5. Ultrasound - shows indirect symptoms of pathology.

Laboratory diagnostic methods include:

  1. Geogram.
  2. Blood analysis.
  3. Coprogram.
  4. Conducting other analyzes.

The diagnosis method is determined by the attending physician, therefore, it may differ from that described.

Treatment

Treatment of ulcerative colitis of the intestine is selected individually for each patient. Its essence can be directed to the following processes:

  1. Stopping bleeding.
  2. Normalization of water and salt balance.
  3. Reduction of mucosal trauma for rapid recovery of intestinal tissue.

Personal choice of treatment allows you to achieve the shortest treatment time, and also has a low toxic effect that can appear from taking pills.

If the medications do not bring the proper results, the main signs do not go away, then the doctor prescribes an operation. Among them, there are 3 main types for the treatment of colitis:

  1. Palliative - the essence of surgery is not the complete removal of the mucous membrane and parts affected by ulcers. A similar method is used when less than 60% of the intestine is affected, and the foci themselves are concentrated in different parts.
  2. Radical - methods are used for severe damage to the intestine, when there is no way to restore tissue. In this case, the removal of certain parts is carried out, as well as the restoration of the integrity of the residues.
  3. Reconstructive - the operation consists in the complete removal of the intestine and the installation of a prosthesis.

After surgery, patients are expected to take medications and take a long time to recover. The basic rules are as follows:

  1. Use of dietary food.
  2. Taking antibiotics and medications to normalize the intestinal flora.
  3. Infusion treatment, which consists in normalizing the water-salt balance by introducing fluid into the body. This process eliminates dehydration and restores carbohydrate stores
  4. Taking astringent medications to normalize the stool. Doctors may prescribe herbal medicines or synthesized medicines.
  5. For maintenance, corticosteroids are used.

The main indications for surgical treatment will be the following indications:

  1. Perforation.
  2. Bowel obstruction.
  3. Abscess.
  4. Bleeding and fistulas.
  5. Cancers.

Medication

Conservative treatment consists in eliminating inflammation using various drugs. For treatment, non-steroidal drugs, corticosteroids, and immunosuppressants are most often used.

Medicines are used in a certain sequence, and if there is a good result, then additional funds are not prescribed.

The main medications for therapy are as follows:

  1. Acetylsalicylic acid. Component-based medicines can affect the intestinal mucosa in certain areas. This group may include the following medicines: Pentasu, Sulafalk, Sulafalazin. It is forbidden to use Aspirin for colitis, since ordinary acid can increase the intensity of the symptoms of the disease.
  2. Corticosteroids. They are used in small courses, about 3 months each. Their use allows you to achieve a phase of remission, and also reduces the exacerbation of the disease. These medications reduce inflammation, but long-term treatment can cause side effects, including increased sweating, hair growth, sleep disturbances, and a weakened immune system. With prolonged use, type 2 diabetes, hypertension and other diseases can develop due to the failure of calcium absorption. Growth may be stunted in children. Such medications are recommended for use in severe disease, when other drugs do not give results. For treatment, metiprednisolone or prednisolone is suitable.
  3. Immunosuppressants Medicines relieve inflammation, and the main action is to suppress the immune system. As a result of this, there may be infection with infections, which means that the funds are used in short courses, no more than 2 months. During therapy, it is recommended to use Cyclosporin, Infliximab, Methotrexate.

In addition to the drugs described, doctors can use other drugs if there are characteristic symptoms, such as stool disturbances, temperature rise.

Additional medicines include:

  1. Antibiotics A group of drugs is used for severe inflammation, when the patient's temperature rises, there is an infection of the intestine with bacteria. The choice of drugs is carried out individually for each person, depending on the severity of the disease and personal characteristics. With ulcerative colitis, drugs called Tienam, Tsifran, Ceftriaxone have proven themselves well.
  2. Remedies for diarrhea. You can use pills of this type strictly as directed by the doctor. Inflammation and stool-holding drugs can cause severe toxic effects on the body, causing megacolon. This condition can be fatal if not provided with an ambulance. All tablets from this group are supplemented with Loperamide or Immodium.
  3. Pain medications. They, too, should be appointed by the doctor individually for each person. For treatment, it is not recommended to use non-steroidal drugs, which can relieve pain and inflammation. Thus, it is necessary to stop taking Ibuprofen and other medicines based on it, as well as from Aspirin. The described tablets can aggravate the condition, cause an increase in symptoms and the appearance of side effects from the digestive system.
  4. It is imperative to use medications to restore iron levels. With colitis, anemia often begins with severe symptoms, and blood loss occurs. For treatment, iron tablets are used, which are used alone or as vitamin complexes.
  5. With ulcerative colitis, diarrhea develops, so patients become severely dehydrated. To normalize the water-salt balance, it is necessary to use rehydration solutions, drugs that contain potassium and magnesium.

If there are additional diseases that complement ulcerative colitis, then doctors can use a number of other medications to maintain the health of the intestines. They are selected based on diseases.

It is imperative to use vitamin complexes, which will contain trace elements, especially calcium, as well as vitamins A, C, K.

With the appearance of pus and complications associated with it, the presence of an infectious infection, doctors prescribe systemic antibiotics.

Only medicines for such a serious illness cannot cure a person. During remission, you will need to use physiotherapy.

They are recommended for use when there is no bleeding or pain. Best applied:

  1. Alternating current.
  2. Diadynamic therapy.
  3. Interference therapy.

With mild and moderate ulcerative colitis of the intestine, outpatient treatment is carried out.

In other cases, with a severe course of pathology, therapy is carried out in a hospital setting, since all interventions can have serious consequences that can threaten the patient's life.

If the treatment with conservative methods is chosen correctly, with the timely detection of pathology.

The patient himself strictly follows the doctor's recommendations and dietary nutrition, and also supportive treatment is used during the period of remission, then the quality of life can be significantly improved.

It is very difficult to achieve complete recovery in ulcerative colitis; it will be only in the early stages with timely assistance.

Diet food

In ulcerative colitis, the main treatment is always supplemented by proper nutrition. For this, a diet is used that allows you to take the load off the affected part.

The diet itself is very strict and some patients will need to follow it throughout their lives, be sure to abandon prohibited foods.

The basic principles of nutrition are as follows:

  1. All dishes and products must be interrupted, eaten in chopped form. It is better to cook only by steaming or boiling.
  2. Completely abandon hot sauce, fatty, alcoholic and spicy.
  3. It is recommended to cook plant-based foods because raw ingredients are high in fiber, which can negatively affect bowel function.
  4. In case of exacerbations, it is necessary to use more liquid dishes, grated cereals, boiled in water. It is not recommended to use buckwheat as it enhances peristalsis.

Among the permitted products are:

  1. Boiled poultry, namely chicken or turkey. Other meat can be veal, rabbit.
  2. Dried fruits, especially healthy blueberries, pears.
  3. From cereals, it is recommended to use semolina, oatmeal, white rice.
  4. You can use small amounts of butter.
  5. Bread is not allowed, but crackers are allowed, use only white bread.

Prohibited foods and products include:

  1. Any flour products and sweets.
  2. Rich first courses, milk porridge and soups.
  3. Fatty types of fish and meat.
  4. Preservation of any kind.
  5. Dairy products, including kefir.
  6. It is not recommended to use pearl barley, buckwheat, wheat groats from cereals.
  7. Any kind of snacks, sausages, smoked meats.
  8. Fresh fruits and berries.
  9. Sauces.

This is only a fraction of the permitted and prohibited products. It is recommended to consult with your doctor for a complete list, which may vary depending on the severity of the disease and the individual characteristics of the organism.

To understand the diet, you need to familiarize yourself with the approximate diet for colitis. Below is the menu for 1 day:

  1. For breakfast, we recommend porridge in water, in which 1 tsp is melted. butter. A steam cutlet is made for the side dish, washed down with rosehip broth or other folk remedies.
  2. You can have a snack before lunch with cottage cheese and berry jelly.
  3. For lunch, make a soup of potatoes with the addition of meatballs, for the second, prepare a rice casserole with the addition of minced poultry, wash down with uzvar.
  4. At afternoon tea, drink tea with breadcrumbs.
  5. For dinner you can stew vegetables, steamed fish cutlet and a light hour.
  6. Eat a baked apple a couple of hours before bedtime.

This food is suitable for colitis during remission. The main positive properties of proper nutrition in case of illness:

  1. Rapid restoration of the digestive system, due to which the stool is normalized.
  2. The effect of medications improves, since the protein rate is restored, and wound healing on the intestinal mucosa improves.
  3. The body receives a sufficient amount of nutrients, metabolic processes are stabilized, and the energy supply is replenished.

Knowing the main products that can and cannot be used, you can make an approximate menu for a week or a month, and then let the doctor see it, who will make adjustments if necessary.

Folk remedies

Alternative methods of treatment can be used to treat ulcerative colitis, but they can be beneficial and effective only at the beginning of the development of pathology.

Each remedy must be discussed with a doctor before use, so as not to aggravate the condition. It is best, according to patients, to use fasting, as well as proper nutrition.

Avoiding all animal products and switching to raw foods is ideal. Good results with colitis are given by means based on herbal ingredients.

To do this, use:

  1. Potato juice.
  2. A decoction based on alder cones.
  3. A decoction of wormwood or yarrow.

Other recipes include:

  1. Infusion of watermelon peel. For cooking, you need to dry the crust of the watermelon and add 500 ml of boiling water to 100 grams of the crushed product, let it stand for 1 hour, filter. Drink infusion 5-6 times throughout the day, 100 ml. The drink can relieve inflammation in various forms of colitis.
  2. Potato juice. It must be drunk to reduce inflammation, it can be made from grated raw potatoes, from which the juice itself is squeezed. Drink it 30 minutes before a meal in small portions.
  3. Infusion of herbal collection. For a drink, mix peppermint, chamomile, and cinquefoil in equal portions. For 1 tbsp. collection, add 250 ml of boiling water, leave for half an hour, then strain. Drink the medicine 250 ml 3 times a day.
  4. Bird cherry. Such a herbal component is recommended for exacerbation of colitis. For the broth, add 1 tablespoon to 250 ml of boiling water. cherry flowers, insist and drink 3 times a day, 50 ml.

Folk remedies can be used in conjunction with medicines for the fastest recovery and removal of symptoms of pathology.

Prevention and prognosis

Preventive measures include using the correct diet, as well as frequent medical examination.

All diseases of the digestive system that pass in the chronic phase must be treated promptly.

It is necessary to follow the clear rules that the attending doctor indicates, to use medications, if necessary, to maintain normal well-being.

The longer the disease is not treated, the more difficult the condition will become, and the prognosis only begins to worsen.

Without therapy, the disease develops rapidly and becomes much more difficult to treat. At the very beginning of the disease, the prognosis will be favorable with the use of proper nutrition and medication. Full recovery can be in 30% of people, and without surgery.

Without proper measures, a total form of colitis develops, and 50% of patients cannot bring the disease into a state of remission.

Ulcerative colitis of the large intestine is a serious pathology that requires compulsory therapy with the help of a qualified doctor.

The diagnostic process cannot be postponed when characteristic signs of the disease appear and it must be remembered that with an exacerbation, the intestine is quickly affected, and the disease can cause cancer and other complications.
